ECG showing frightening ST elevation in the anterior leads, and no terminal T wave inversion (such inversion would suggest a subacute/evolving MI, not acute)
There are also deep q waves in V1-V3.

A past case at OSH. Undiff syncope, but saw this ECG and knew I had to do a POCUS echo right away. 1/2
ECG with mild sinus tachycardia and with T wave inversions in leads III and V1-V4. The T wave inversions strongly suggest right ventricular strain, such as a large pulmonary embolism would produce.
