A new GP contract has recently been announced which will affect how referrals from your GP work in the UK from the 1st April… There will be a significantly heavier reliance on GPs using “advice and guidance” - reaching out to specialists for their input rather than making a referral in the traditional sense. The motivating factor here is very clearly to reduce outpatient waiting lists, which of course it will do - but as this change only increases the burden on GP surgeries, taking up more GP appointments to action that advice, you can expect to find it even harder to see your GP moving forward.Lots of smoke and mirrors here, I suspect, to make the numbers “look better” but without actually improving patient care - or, likely, patient outcomes. How do you feel about this change?Personally I suspect that using “advice and guidance” will also create issues for patients with chronic, rare or complex conditions - and most likely for female patients - as the change means that any implicit assumptions or bias that may have crept into the GP’s original request for advice and guidance will be passed along to the consultant as “the full story”. You will no longer have the opportunity to state your own case… NHS England have apparently assured doctors that referral pathways following Jess’s rule will continue as normal - so if you have seen your GP three times or more for the same (undifferentiated) condition, you can ask your doctor to Reflect, Review and RETHINK - so this may become the best pathway to follow until the situation settles. If you are a GP - please weigh in on whether this change will be of benefit to your patients? Or do you feel this is just another step away from patient-centred care?#medicalgaslighting #womenshealthmatters #doctorsappointment #chronicillnesswarriors #autoimmunediseases Keywords:NHS ReferralsJess’s Rulemedical misogynyPOTS / MCAS / EDSLong CovidChronic FatigueWaiting Lists Rare diseaseView all 4,252 comments
