I use Lidocaine for shorter more minor arthroscopic procedures ..sometimes with suprascap block ..otherwise 0.75% L 👍 I agree , certainly quicker than time taken to position etc

I use only ropivacain 0,75% for awake shoulder procedures. Never had any problems concerning onset time. Surgeons are not fast enough in patient positioning 😉.

Did 5 awake shoulder procedures yesterday. Interscalene block 0.75% Levo 6-10ml Plus sup Cx plexus 0.75% Levo 5ml I’m pretty convinced the onset time is equivalent to Lidocaine 1-2% …so no mixy mixy for me 🤪

Just relistened to both episodes back to back chilling after my morning run. I LOVE the ED perspective on RA…those subtle nuances of WHY they do the blocks and the consideration given to “Trajectory” of the patient. To quote The Clash: “So you got to let me know Should I stay, or should I go?”

We’ve just ordered these little beauts from Pajunk as stock for paeds, hands and ortho. 25g 50m. Lovely to use for ankle blocks, brachial plexus, genics plus baby blocks. Fine tip allows greater precision around nerves. Also great with stimulator for teaching/defining nerves👍

1/The U.K. is soon to become 100% compliant with NRFit devices. Whilst there remains the possibility of drawing the wrong drug into the syringe, the yellow colour is a clear visual reminder of the possibility. Questioning trainees, they would feel very uncomfortable reverting to Luer. Next step?

Anaesthetists: Is it just me or does everyone think is a little strange that anaesthetists have to take significant amount of time snapping open glass ampoules, needling vials etc and drawing up the contents, risk/time/tasks. Maybe more drugs should be readily available in prefilled syringes??
