I have just finished reading it, and will probably re-read. I can't disagree with any of it. Sobering stuff. Although I do think the 'old' system of (junior doctor) recruitment was perhaps more open to favouritism? That's not to say the current system is not severely flawed. My review on Amazon:

It reads easily, like a narrative essay in short chapters, and in a way that is accessible and relevant to those outside of the medical profession as well as those in it.

It blends the factual with personal perspectives informed from decades of experience.

I would actually suggest reading Chapter 11 - the concluding chapter - first. And then again at the end. It not only summarises the content of the book but also, in my view, provides context to the current resident doctors' disputes and why striking is one of the only tools we have left."

Great article, brilliantly written. We also need ‘space’ to think. But instead, we see: •erosion of SPA time •disappearance of office space •increasing unnecessary administrative workload (that would be better done by skilled administrators)

interesting post from @andrewlearmonth So why not a different approach that's silo agnostic & vendor neutral? An approach that's more, flexible & intelligent? One where primary & secondary care share the same data repositories but in different ways, with real access control across all geographies

Agree, provided that it is the clinical record - interventions, obs, diagnosis, plans, examinations but needs to be vendor neutral, perhaps curated by clinicians, and reach across all boundaries where the above occurs. Access to such records could be managed in a far more intelligent way too.
