I have this in my calendar. It was obviously of some significance when I put it in (possibly years ago) but I have zero recall of why I would have done.

In Canada there are many more applicants than places. Canada does OK ish in recruiting applicants from rural/marginalised/lower socioeconomic. Could still be a lot better. But it’s the places that’s the issue.

Except you have to find them. I am on call three hours away from home this weekend as they have been unable to recruit more anesthesiologists here, in part because they have not been able to recruit more surgeons and OBGYNs. Can’t just make us out of thin air.

Which would indicate that those who got myocarditis from the vaccine were the ones who actually benefited the most and therefore were more in need of vaccination. If this were all true.
111
Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
Thus, what I’m interested in is the flow on from your question: would those who got vaccine induced myocarditis have got more severe MIS/myocarditis if they’d encountered the virus unvaccinated.
110
Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
Absolutely… the relative risk is not even comparable. The smart evidence based decision was/is to vaccinate. However we are not rational beings. Acts of commission are viewed more harshly than acts of omission. Thus the one death “associated” with the vaccine is “weighted” more harshly.
110
Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
3. ..suffer more severe myocarditis from disease. Thus the vaccine reaction saved their lives as it potentially indicated the more severe reaction they risked if infected. But that’s purely from my family physician, non immunologist lens. I keep hoping someone who knows will answer 🤷‍♂️

Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
2. With reference to your second question I have intermittently been trying to find that out. Given the mechanism of myocardial injury described in both COVID and vaccination my hypothesis would be that, those who suffered myocarditis from the vaccine would (be genetically predisposed to?) /3

Difficult to tease out the causation versus association in the setting of meeting the criteria for rheumatic fever. The balance of the evidence presented in the paper suggest the cause of death was rheumatic carditis and the mRNA was a “temporal” association only. And it was one not plural. /2
100
Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
3. The paper reporting the death: pubmed.ncbi.nlm.nih.gov/38400177/ “An antistreptolysin O (ASO) dosage showed high titers. The presence of arthritis, elevated ASO, and carditis fulfills the criteria for rheumatic fever.” _Association_ with one death but evidence of another cause.

2. “numerous worries over its adverse effects have emerged during the pandemic alongside various case reports, Reference Tan, Huang and Mei44–Reference Valore, Junker and Heilmann48 summarised in Table 4.” In table there’s one paediatric death. The rest were listed as full recovery. /3

Dr Paul Mackey @auscandoc.bsky.social · 28/09/2026
I dig a bit deeper into this statement by checking the references. “in children, and there have been rare, documented vaccine-associated deaths in this population.” Refs44-48 seem to be where they are deriving this statement from. /2
