C3G has entered its championship era!! It is no longer just “interesting biology” but one of the most exciting treatment stories in nephrology. Loved writing this commentary on why complement-targeted therapies could be a real game changer for patients. @nephmadness.bsky.social

Absolutely Taylor! I am with you on the aspect of patient voice. Will review the points in the article. We were looking at it from purely a scientific perspective since there’s a lot of discrepancy in the literature that has affected patient care & research in the field.

For that reason, the proposal was not to remove the name aHUS altogether. I think the physicians, patients and pharma need to come together as a next step to make sure everyone’s interests are aligned without compromising scientific advancement.

Anuja Java @anujajava.bsky.social · 19/04/2025
However I also see how this can be from a pt’s perspective when they have been diagnosed with a certain disease and now unsure what to call it. Moreover, since the drugs are approved for “aHUS”, it has created some unrest.

With advancements in understanding of etiology & ongoing understanding of the role of complement in various TMAs, it is important to move to a more etiology based classification so that appropriate & timely t/t is available to those who need it and avoid unnecessary treatment in patients who don’t.
120
Anuja Java @anujajava.bsky.social · 19/04/2025
Similarly, aHUS was a term given to a disease when it was not HUS. So it describes what it is NOT rather than what it IS.
110
Anuja Java @anujajava.bsky.social · 19/04/2025
From a scientific standpoint yes absolutely it is a step in the right direction. This is similar to why C3G is not called MPGN anymore. We still struggle with the term FSGS because we don’t understand the etiology fully. We have called diseases by what is seen on the biopsy.

Doesn’t concern me. If anything, it spares the MAC, so risk of infection potentially lower compared to other anti-C agents like Ecu which we use along with IS.

I am literally just thinking aloud here:).
120
Anuja Java @anujajava.bsky.social · 18/01/2025
This concept is similar to donor specific Abs after transplant. Presence of a DSA doesn’t always mean it’s going to cause AMR. So how aggressive do we need to be to get rid of the DSAs and below what titre do we feel comfortable. Ultimately it is determined by clinical picture + functional testing.
140
Anuja Java @anujajava.bsky.social · 18/01/2025
I further worry about the balance between suppressing C3 enough to prevent convertase formation that prevents Nef formation but not enough to still have C3b for his opsonization & other protective function. I don’t know what that balance is and what is threshold for each.
230
Anuja Java @anujajava.bsky.social · 18/01/2025
I think this is imp because if you suppress C3 to prevent formation of a convertase, you may not have enough antigen presentation to B-cells to produce Nefs but not sure it would prevent production of naturally occurring C3 convertase Ab which you may still detect if you are using a binding assay.
120
Anuja Java @anujajava.bsky.social · 18/01/2025
So it’s important to first know what methodology is being used to test presence of Nefs because if one only uses binding assays, then it is not clear whether it is a benign C3 convertase IgG or a pathogenic nef. Therefore a functional assay that detects what that IgG is doing can determine it
