Another question I came here with was: how effective are the whistles and honking? Does it stop abductions? And the answer is, surprisingly to my cynical self, yes, and often. I have talked to so many people who have successfully interrupted kidnappings.

ICE has its own Health Services Corps, which it operates in 17 large detention centers, but an increasingly large majority of people are held in private detention centers where the healthcare is operated by contractor or subcontractor, where ICE basically just manages the insurance claims.

ICE Health Care System
IHSC staff protect our homeland by stopping the spread of infectious disease and supporting ICE removal operations.
IHSC staff protect our homeland by stopping the spread of infectious disease and supporting ICE removal operations.
IHSC is committed to providing the necessary medical intervention for aliens to reduce global disease and support the safe apprehension, enforcement and removal of aliens involved in immigration proceedings.

In Fiscal Year (FY) 2025, IHSC provided medical services to more than 171,000 detained aliens housed in 17 facilities throughout the nation. In FY 2025, IHSC oversaw compliance with health-related standards for over 266,000 detained aliens housed in 217 non-IHSC-staffed facilities, totaling over 50,700 beds.

IHSC Headquarters provides guidance and support to IHSC-staffed facilities. While IHSC does not directly provide medical services in non-IHSC-staffed facilities, IHSC audits the facilities’ compliance with national detention standards, as required by each facility’s contract with ICE.

“The ICE Health Service Corps staff comprises the highest caliber of health professionals who have chosen detention health as a noble and rewarding career,” Smith said. “Our team is second to none.”

ICE Health Service Corps (IHSC):
IHSC delivers health care to noncitizens at 19 designated ICE facilities and provides high-quality medical, dental, mental health, and public health services to noncitizens in ICE custody. ... In situations where services cannot be provided by an IHSC-staffed facility or a contracted detention facility, off-site medical treatment is provided. ... Through field medical coordinators, IHSC provides case management and health oversight for noncitizens and is responsible for initial noncitizen screenings for infections ..., and includes comprehensive chronic disease management ... In FY 2023, IHSC provided services to 131,359 noncitizens for a total of 1,264,680 encounters. The following procedures were conducted: 87,763 physical
examinations; 136,386 intake screenings and 23,967 emergency room and off-site referrals. IHSC also conducted an additional 14,149 dental visits,
22,379 urgent care visits, 150,683 sick call visits, 49,651 mental health interventions and filled 266,109 prescriptions.
Third Party Medical Care Provisioning: Third Party Medical Care refers to the “off-site” medical treatment ICE noncitizens receive from State and local medical providers in U.S. community hospitals and clinics. Off-site medical claims are submitted to ICE’s partners at the Veterans Affairs (VA)
Finance Services Center (FSC), where the costs on those claims are processed, adjudicated and approved by IHSC, and paid by ICE. When treatment is “on-site” health services are
received noncitizen in ICE custody either through IHSC, or the medical provision is provided by a contracted detention vendor. In FY 2023, IHSC
provided oversight and management of the off-site claims adjudication process for over 162,976 detainees housed in 163 non-IHSC-staffed facilities,
totaling over 51,000 beds. At these locations, the contracted detention vendor or its subcontractor manages the health care provision.
