Community health centers served 32.7 million patients in 2025, with 9 in 10 patients living in low-income households. Medicaid was the largest payer, covering nearly half of health center patients and accounting for 46% of total health center revenue. on.kff.org/3ZdWYfG

Georgia is facing a Medicaid funding shortfall because the state has to adapt their work requirement policies to new federal standards, which are more lenient, vis-a-vis exemptions, than what the state had implemented and budgeted for in their partial expansion (receiving only standard FMAP).

Some red states are prohibiting people from attesting to being too sick to meet Medicaid work reqs, meaning people will need to gather medical paperwork to prove frailty — and states will need to be ready to process it quickly and accurately. Both of those are cracks people will slip through.

Worked with @bobjherman.bsky.social on the data for this piece. Here’s the chart I think is the showstopper: Over the last 40 years, the amount of money spent on employer-sponsored insurance premiums skyrocketed by nearly 1000%, well above baseline inflation.

"States are going to be asked to make a determination using information that doesn’t exist in their systems." Still lots of confusion over whether/how states can automate frailty determinations for Medicaid work requirements. And states don't have the luxury of time for CMS to figure this out.

States have some ways to tell whether someone has a diagnosis & we've heard states are working on code lists & claims data they can use to try to identify people automatically who would have this exemption...But adding an element of ‘unable to work’ — nobody knows where that data will come from.
