Prevalence of anticholinergic medication classes by dementia status among veterans in long-term care. Among residents with dementia (n = 6638), first-generation antihistamines (29.5%), second-generation antipsychotics (30.6%), and bladder antimuscarinics (20.2%) were the most commonly used classes. Among residents without dementia (n = 8436), first-generation antihistamines (34.4%), bladder antimuscarinics (23.7%), antidepressants (17.7%), and antiemetic/antivertigo agents (11.9%) were most prevalent. Percentages were calculated using the total number within each dementia stratum. Class-specific totals exceed 100% because individuals may receive medications from multiple anticholinergic classes.

How do we help older adults who develop new disabilities after a critical illness? This small qualitative study suggests: - promoting ⬆️spiral of resilience & community reintegration - counteracting ⬇️spiral of demoralization after critical illness www.ajgponline.org/article/S106... #medsky
The upward spiral of community reintegration. [A] Cross-section of the virtuous spiral of community reintegration with structural support using 4 strategies. [B] Longitudinal view of the virtuous spiral of community reintegration with structural support using 4 strategies. The figure represents how the health or social care system (support column) can deliver strategies (spokes) that link to community reintegration outcomes/health states (spiral). Maroon: Develop patient and family skills à manage symptoms of underlying impairments at home; Orange: Re-establish competence to perform healthy habits and routines; Blue: Identify & support key roles and relationships à stay connected even amid change. Green: Problem-solve the physical and social environments à Enable ongoing participation in meaningful activities. [C] Problem-identification using the virtuous spiral of community reintegration. Problems are any subjective or objective barriers to continuing to make progress toward, or to maintain, community (re) integration. They can occur anywhere on the spiral. Hypothetical Example Problem: A patient is having difficulty re-establishing a rhythm for doing laundry and meal preparation. [D] Troubleshooting problems. Troubleshooting involves a root cause analysis of why things broke down and the main impacts of the breakdown. Because the spiral is interconnected, both the root cause and the impacts can occur in any or multiple quadrants. Thus, all quadrants should be assessed to identify cross-cutting issues. Example Troubleshooting: On further questioning, the problem with laundry and meal preparation (orange quadrant) is that she’s exhausted because her sleep is disrupted. Sleep is itself a routine (orange quadrant). The question is, what is disrupting her sleep? While multifactorial, the main driver seems to be insomnia related to unrecognized trauma symptoms (maroon quadrant). The trauma symptoms have also led to some irritability that is impacting her relationships (…

Inching closer to precision medicine in older adults at risk of #dementia: In this secondary analysis of PACT-MD (cognitive remediation + transcranial direct current stimulation to slow cognitive decline), thicker cerebral cortex = more likely to benefit. www.ajgponline.org/article/S106... #medsky
X axes represent the study follow-up (in years) and the Y axes represent the Z-scores for global cognition. Note that panels use different y axis scales; this should be considered when comparing effects across groups. The dots at time 0 represent the actual study baseline (i.e., the data are not estimated by the model); the first predicted responses represent the two-month assessments, and the following predicted responses represent the annual assessments. The blue lines represent the active intervention and the red lines represent the control intervention. While brain measures are analyzed as continuous variables, they are split by median on these figures. Overall cortical thickness showed a significant moderating effect (A, B; likelihood ratio test p=0.015). Overall FA did not moderate the effect of the intervention on global cognition (C, D; likelihood ratio test p=0.139). ROI cortical thickness also showed a significant moderating effect (E, F; likelihood ratio test p=2.184x10-6).

1- What's the relationship between vitamin D supplementation & falls in older adults? It's complicated. ⬆️doses of vitamin D are associated w/ ⬆️executive functioning, processing speed & physical activity. agsjournals.onlinelibrary.wiley.com/doi/full/10.... #medsky
The speed at which the central nervous system processes sensory information, a critical determinant of falls and type of resulting fractures, is dependent on sufficient levels of the active metabolite of 25OHD. Healthy individuals are able to make an appropriate and timely postural response to a perturbation of balance. With the slowing of central processing speed, that response, flexion at the knees and hip and extension of the arm(s), is delayed. The individual falls to the ground with the arm or arms extended to break the fall resulting in a wrist fracture. With further slowing of central processing speed, little or no postural response is generated before impacting the ground. That impact force is likely then to be directed to the femur greater trochanter, resulting in hip fracture.
