It's worth having a discussion about what should happen when trials do this and whether we should be collecting additional data from those investigators to make sure the trial analysis is not compromised by these changing outcomes (e.g., unblinding date, reason for change).

The success of this tool at scale, with relatively cheap models, suggests that this could be a real-time surveillance tool for NLM to monitor change to primary outcomes when they happen (maybe they are doing this in recent years).

This number was higher than we would expect, and higher than we want. That isn't to say that they are all problematic in a way that would, say, invalidate all these trials. But the number being this high means that problematic changes may be harder to catch in the huge numbers of changes happening.

In summary, we found that over 30% of trials had at least one of the three change types after the trial had started. We did find signal that these changes are happening less in recent history, a good sign. We found associations with industry funding and device trials and higher outcome switching.

clinicaltrials.gov does allow for updating registrations -- it has to, for things like updating enrollment counts, adding results, and listing trial publications. However, it also allows for modifying primary outcomes, even after the trial start date.

In the US, clinical trials must be pre-registered on clinicaltrials.gov. This means that you have to publish your protocol, including the metrics you will use to measure success, ahead of time. This reduces bias by preventing investigators from picking metrics that make the data look better.

We have a research letter published in JAMA Network Open that we're excited to have out there. We used LLMs to study outcome switching in clinical trials, and found (perhaps) surprising amounts of it. Quick thread on 1) What is outcome switching? 2) why is it bad? 3) What did we do to detect it?

Maybe this is a minority opinion, but I don't blame athletes who choose to go. These guys aren't geniuses and don't all follow politics. Being invited to the Whitehouse still feels like an honor to them. They are also not responsible for a president saying crazy things while they are there.

I appreciate it. I think there is a potential benefit at scale where it's easier to live car-free even for people who "need" a car for groceries and such. But I'm worried when I visit Atlanta and I see armies of Waymo vehicles driving around downtown empty.

I think framing this in terms of homelessness is not productive. But it's not that hard to find RVs with multiple bikes around them. They are stolen to be sold. Regardless of the reason for theft, if SPD doesn't take it seriously there will be a lower ceiling on how many normies we can get riding.

I guess you could say that about any public good that hasn't been auctioned off to advertisers yet. Hard to measure the good it does and maybe a matter of personal preference but I think we go way too far in the US in selling off public space and find their attitude refreshing.
