Just discussing the phenomena of 'pretendians', those who create false claims of Indigenous identity for a variety of reasons: financial gain, access to certain positions, fulfilling personal needs, living out family mythology. The Yellowhead Institute has a good report on this in academia.

You've maybe seen the videos/photos of that terrifying fly-by at the rugby match in Cape Town by AirLink an E195 and E190. The airline has since released a statement that declared the event was done as approved by the aviation authority. The unwritten subtext "go away we know what we're doing".

A small example of how technical codes progress to improve safety. Just had a reno that included relocating some receptacles. Having to meet new code is usually at the discretion of the AHJ. In the rough inspection the codes person identified a new receptacle was required in the laundry room.

This is well reflected in military history. Getting a hot meal out to forward deployed troops right before a major assault. Holding a semi-formal Christmas dinner in the field (great photos from these in archives). This one in Ortona. This is what good senior leaders do.

like the faulty AOA vane in the Ethiopian Air 737 Max crash. If you call that 'root cause' what motivation is there to look at the socio-technical factors that allowed a single sensor flight-safety-critical system to be certified? It requires looking at the entire system. From Nancy Leveson:

as it’s consistent in all complex systems. Exceptionally rare to see an organization accept moral culpability - quite the opposite. Columbia’s inquiry needed to fire a block of foam at the leading edge of a shuttle wing, as the senior people at NASA refused to accept it would fail that way.

from a stable system, operating as designed, to one outside its design parameters. For a boiler this is catastrophic. Rasmussen described this in terms of movement of an operating point bounded by performance/workload/economic boundaries. It’s a useful cognitive model of complexity.

Well, I wonder what could be behind this? Photo radar was pulled early on, but radar wouldn’t deter reckless driving or poor judgement. And there have been traffic calling measures installed in residential areas, so positive mitigations increasing. www.cbc.ca/news/canada/...

But the term carries with it another appeal to authority. As my colleague noted, one of the issues under discussion was that 'clinician scientists' have special insight into the interpretation of evidence through EBM. This is unhelpful, and is used frequently imo to silence challengers.

I'll post about the talk afterwards. The context of this interaction was a 'clinician scientist' making some assertions about evidence-based medicine. I pushed back and noted that the default presumption of expertise I've encountered in EBM is unhelpful and harmful. I got this response.

Looking at the photo of the recent bus fire, and assuming the battery is low down somewhere between the axles, I’d agree the battery didn’t burn. Fire low burn looks like bottom of the driver’s compartment. Unless the battery is up front. Next to no info on the bus design out there.

The PHA still gives the CMOH that authority but there’s a whole section about the minister and orders in council now. I was surprised to hear her declare that cabinet made the decisions after reading the PHA. Not many reasonable paths of interpretation supporting deferral to cabinet.
