Congratulations to first author Keeley Ravellette., whose undergraduate honors project at the UArizona launched this research. This project reflects the collective efforts of an outstanding multidisciplinary team and long-term collaborators across physiology, cardiology, and pulmonary medicine.

📈 Upward shift of the ZC-mPAP relationship (PVR > 8.5 WU) suggests an increase in the proximal arterial stiffness (E) for a given pressure. 🧰 Impedance is a potential tool that could be used to better understand function changes in the proximal and distal vasculature in patients with PAH.

💻 Functional proximal arterial mechanics were evaluated using the ZC to mean PA pressure relationship (ZC-mPAP). ⚖️ The ZC-mPAP relationship allows us to disentangle pressure-induced from structural changes in ZC after normalizing for pressure.

🫁 Impedance metrics of resistance (Z0) and vascular stiffness (Z1 and ZC) increase with disease severity. 🫀 Impedance metrics correlated with arterial elastance (Ea) and RV contractility (Ees) but characteristic impedance (ZC) did not correlate with RV-PA coupling (Ees/Ea).

Pulmonary vascular impedance provides a more comprehensive assessment of RV afterload in PH. In this manuscript, we used standard clinical measures of PA pressure by right heart catheterization and RV out flow track velocities by echo to calculate pulmonary vascular impedance and show:
