A race-based kidney function formula delayed transplants for Black patients for decades. A 2023 policy restored lost wait time for 21,000+ candidates, and transplant rates rose. New evidence that reparative policy works. 🔗 doi.org/10.1001/jama...

Removing race from kidney function algorithm helped more Black patients access transplants
Removing race from the eGFR had a big impact, resulting in 5.3 more kidney transplants per 1000 Black candidates.

Dashed line denotes the date of initial OPTN Wait Time Modification Policy implementation on January 5, 2023. The shaded gray box and points plotted without a trendline from January through March 2023 denote the implementation washout period, which we prespecified based on the low number of wait time modifications approved throughout this time (Figure 1). To illustrate pre- and postimplementation trends, trendlines based on predicted probabilities from our interrupted time series models are also plotted.

National implementation of Organ Procurement and Transplantation Network wait time modifications for Black kidney transplant candidates affected by race-based eGFR was associated with increased transplant rates, mainly via higher DDKT rates. ja.ma/4ujvsLS
Line graphs titled "Trends in Overall Kidney Transplant Rates by Race/Ethnicity and Dialysis Status, January 2022 to June 2025," show monthly kidney transplants per 1000 waitlisted candidates for Black vs. Comparison cohorts across Overall, Predialysis, and Postdialysis.

In a new @nejm.org article, @rohankhaz.bsky.social et al. call for a shift away from race-based evaluation of occupational pulmonary impairment, noting that race-neutral PFT equations better predict clinically meaningful outcomes like respiratory symptoms. Read more: tinyurl.com/25rtkmsk

💡 FXB Affiliate and #health #equity advocate @rohankhaz.bsky.social and his team published a study on @nejm.org describing the history of how race and pulmonary function testing quantify lung function impairment and outlining a path toward more equitable evaluations. Read ➡️ tinyurl.com/49sahazm
Race-based pulmonary-function test (PFT) guidelines were taught to clinicians for decades, despite questions about their validity and implications of use. Within medicine, a growing consensus gradually emerged for ending inaccurate and imprecise uses of race as a standalone proxy for biologic or genetic traits. Significant gaps persist, however, in implementation of race-neutral PFT equations in the United States and worldwide, because race has long been embedded in PFT interpretation. The path to universally equitable evaluation of occupational pulmonary-function impairment—and to remedying the harms of previous race-based approaches—will benefit from further engagement with patients, medical societies, regulatory bodies, payers, researchers, and clinicians.” Rohan Khazanchi, MD, MPH, FXB Affiliate

💡Childhood trauma and poor mental health are closely intertwined. In 2020, the prevalence of both began increasing among adolescents. FXB Affiliate Rohan Khazanchi co-authored a study assessing adverse childhood experiences as risk markers for broader structural adversity. ➡️ tinyurl.com/mw5hasbv
“Adolescents exposed to adverse childhood experiences (ACEs) experienced mutually reinforcing disadvantages: a heavier burden of mental health symptoms and greater barriers to accessing needed medical and mental health care. ACEs refer to traumatic events that occur during childhood and include physical and emotional abuse, neglect, household dysfunction, and neighborhood violence. They must be reconceptualized as markers of structural—rather than individual—adversity and addressed through structural and policy interventions.” Rohan Khazanchi, MD, MPH, FXB Affiliate

1/ 🧵 Exciting news in MA today -- @massgovernor.bsky.social signed a bill ending mandated #CPS reporting for prenatal #MOUD exposure! This is a huge win for children & families across the Commonwealth, and a product of years of advocacy + research ❤️❤️ #PedsSky #MedSky jamanetwork.com/journals/jam...
Updates policy for infants affected by prenatal substance exposure.  The bill mandates a collaborative approach to conducting comprehensive assessments of how substance use may affect parental capacity and infant safety, prioritizing family unity and access to appropriate support systems. A parent who is stable in recovery will no longer automatically require a report for abuse or neglect.
“Too many families have known the pain of substance use disorder and, tragically, the loss of a loved one because of it,” said Senate President Karen E. Spilka (D-Ashland). “Opioids have stolen life from parents, children, siblings, and friends. The legislation approved by the Legislature and signed by the Governor makes powerful strides towards mitigating this crisis by helping those coping with substance use disorder reduce harm, find treatment, and achieve recovery that lasts a lifetime. I am grateful for the work of Senators Crighton and Velis, my colleagues in the Senate, and our partners in the House and the Administration on this vital bill.”

This is great news. It is common sense that people living in the community (on probation/parole or otherwise) should be able to access their Medicare benefits. We should go even further and normalize health care financing in prison/jails to allow for Medicare/Medicaid benefits to be used there too!
