This is the People’s CDC Weather Report for the week of September 21, 2026! This Weather Report from the People’s CDC sheds light on the ongoing COVID pandemic and other public health crises.

Body text: "We’ve gotten a number of questions about how to explain the differences between CDC and WastewaterSCAN levels. Check out this week’s full report linked in our bio for a description of the differences between the two organizations’ approaches.

Remember, wastewater is an essential tool for tracking changes in COVID and other pathogens over time. The best way to use CDC or WastewaterSCAN data is not to rely on the literal levels they report, but to find the data for the site closest to you and look back to see how your current wastewater value compares to other moments during the pandemic."

Most of the northern half of the country is a shade of purple indicating COVID  infections are “growing” or “likely growing".” Most of the southern half of the country are shades of green indicating COVID  infections are “declining” or “likely declining”. Body text: "According to CDC Epidemic Trend data last updated on September 16th, the number of infections is estimated to be “Growing” or “Likely Growing” in 27 states. It is “Declining” or “Likely Declining” in 13 states, and not changing in 10 states.

These values are estimations based on a formula rather than measured data. It is estimated from daily incident emergency department visits reported through the National Syndromic Surveillance Program. An Rt < 1 does not mean that transmission is low, just that infections are declining. They could be declining from a high level to another slightly lower high level."

Body text: "On September 8, the Mamdani administration in New York City launched a portal of 170,000 previously classified documents related to the attacks, along with memos revealing the city’s mishandling of the response. New Yorkers were led to believe the air was safe, when it remained laden with pollutants for months. Instead, city officials downplayed concerns for a premature “back to normal” approach to avoid the responsibility of guaranteeing clean air. If you haven’t watched it yet, we recommend checking out Democracy Now’s coverage of the 9/11 response. We recognize in this story the same prioritization of “normal” for the sake of global markets and to the detriment of public health that characterized the COVID response."

If you reside outside of these states, call and email your attorney general to encourage them to join the lawsuit.  Tell them to support Medicaid for All which would save both 114,000 lives per year and $1 trillion dollars per year."

COVID levels continue to rise across the country, FDA approves Fall 2026 COVID vaccines, 2 new measles deaths in Pennsylvania, and remembering Dolly Parton.
A data table labeled “COVID vaccines approved for fall 2026.” Moderna has created an mRNA vaccine called Spikevax that has full  FDA approval for those ages 65 and up and approval for those aged 6 months to 64 years who have underlying conditions. Moderna has also created an mRNA vaccine called mNexpike that has full FDA approval for those ages 65 and up and approval for those aged 12 years to 64 years who have underlying conditions. Pfizer has created a vaccine called Comirntay that has full FDA approval for those ages 65 and up and approval for those aged 5 to 64 years who have underlying conditions. Novavax has created a protein vaccine called Nuvaxoid that has full FDA approval for those ages 65 years and up and approval for those aged 12 - 64 years who have underlying conditions. All of these vaccines were made to target the XFG variant.

Body text: "Recently in Pennsylvania, two infants who tested positive for measles have died, marking the first measles deaths in the country in 2026 and the first in Pennsylvania in 35 years. This comes as the measles outbreaks across the US continue to rage and RFK Jr. continues to downplay them. As such, RFK Jr. instructed the new CDC Director to remove these deaths from the CDC’s official tally. 

The Secretary, who has no medical or public health training, claimed the children had underlying conditions so they might have died with measles and not from measles. A tactic the federal government has grown quite comfortable using to minimize COVID deaths."

All of the states that touch the Southern border of the country are experiencing “moderate,” “high,” or “very high” levels except for New Mexico which is “low.” Seven states that touch the northern border of these elevated states such as Arkansas, Tennessee, Oregon and North Carolina are also experiencing “low” levels. Montana and Washington are northernmost states that are experiencing elevated levels, reading at “high” and “moderate” respectively. Alaska, the US Virgin Islands and Guam are experiencing “moderate” levels. North Dakota is gray meaning “limited/no data.”

Body text: "The CDC-based wastewater map was updated on September 3 and depicts levels for the period from August 23 - 29. We can see increases in COVID levels across the country. According to the CDC, Texas is experiencing “Very High” levels, Montana, Arizona, Mississippi, and Hawaii are experiencing “High Levels” though the data from Montana and Mississippi are limited. A school district in San Antonio, Texas closed all campuses for two days just as the new school year began in an attempt to control the uncontrolled spread of COVID in the community.

Regional trend data indicate the West has risen from “Low” to “Moderate.” The South has risen to “Low” with other regions remaining in “Very Low” but trending up. "

Body text: "The US Federal Drug Administration (FDA) has approved four new COVID vaccine formulations for the fall. These vaccines should be available in your local pharmacies soon if they are not already.

Updated COVID vaccines have been repeatedly demonstrated to safeguard against severe illness and hospitalization in people of all ages, because COVID mutates and requires updated, targeted protection. The CDC’s official list of high-risk conditions is extensive and likely includes most Americans, so many under 65 are eligible for these shots and may not know it. Pharmacists may not be aware that “high risk” includes many conditions , so you should prepare for the potential discussion."

COVID levels continue to rise, discrepancies between CDC wastewater and WastewaterSCAN, and states fight the roll-out of Medicaid work requirements imposed by One Big Beautiful Bill.

Body text: "Despite discrepancies between the CDC’s values and WastewaterSCAN’s, both the rise in wastewater elevation that we are seeing in Southern and Southwestern states in the CDC’s map and the “High” to “Moderate” levels in all regions in the WastewaterSCAN map seem as though they could indicate the beginnings of a late-summer wave. 

While COVID trends aren’t exactly the same each year, we can expect cases will continue to increase as the fall school term starts this month and next."

Body text: "This is a good reminder to also look for data coming from your most local wastewater facility. And that “Very Low” levels do not guarantee that you will not be exposed to SARS-CoV-2 or contract COVID in your area. Wastewater trends should be used as guidelines for when to increase precautions, with strategies such as cancelling events, avoiding large groups, or rescheduling certain postponable doctors’ appointments. 

Wearing a properly fitted N95/KN95/KF94 mask indoors and in crowded outdoor spaces, regular testing, and year-round masking policies at hospitals and nursing homes are baseline precautions that minimize viral transmission for everyone, and should be implemented at all levels of wastewater spread."

Notably, the data from WastewaterSCAN tells a very different story. It shows “High” levels throughout most of the country and “Medium” levels in the Midwest."

COVID cases continue to rise across the country with levels ranging from “Very Low” to “High”, Dr. Fauci on trial, cyclospora cases break 20k, police brutality protests in Wisconsin, new COVID research, a new anti-choice CDC Director has been selected, & HIV care and wastewater to be defunded.

Happy LGBTQI+ Pride Month, our new interview segment titled “Take Action Locally,” COVID “low” and “very low” except in Missouri and Guam, Rotavirus and Norovirus at “moderate” levels.

Body text: "According to WastewaterSCAN, national rotavirus levels have decreased to “medium” from their past couple of weeks at “high.” Levels in the Northeast remain “high.” 

Rotavirus is an easily transmissible virus that can cause severe diarrhea and consequential dehydration in infants and children. The current rotavirus vaccine prevents 40,000 to 50,000 hospitalizations annually. Rotavirus is one of the vaccines that MAHA is making efforts to remove from the current vaccine schedule. Recently, Trump issued an executive order for HHS to move to adopt the limited schedule, putting more children at risk."

Body text: "Measles virus has been spreading rapidly across the US for over a year now. The worst outbreaks have been reported in Texas, Utah, South Carolina, New Mexico, and Florida, but dozens of other states have also reported increased cases. 

In 2025, the US saw 2,288 confirmed cases. Just 6 months into 2026, we already have 2,228 confirmed cases.

These outbreaks across the world can likely be attributed to misinformation about vaccine safety, poor access to vaccines, mistrust in government officials, and the dismantling of public health infrastructure by domestic and international parties."

An Interview with ACT UP New York about their public health advocacy campaigns.
This is part two of ACT UP's answer to the question "Since the founding of the NYC free condom program, have you had to do any maintenance advocacy to keep it alive?"

ACT UP New York continues "We had just begun a social media campaign about the defunded free condom program when, at the next meeting with DOHMH (we had arranged follow-up meetings with them until the problem was resolved), they announced that they had found a new source of funding for the condom program and it would resume over the summer.

That experience was a brutal lesson in how quickly the onset of a newer and concurrent crisis like COVID can be used as an excuse for bureaucratic negligence and indifference that is powerful enough to undermine one of an organization’s core missions, in this case, STD prevention through condom distribution."

ACT UP New York answers "HIV and COVID have caused the deaths of millions of people. Much of the early response to COVID was informed by AIDS activists and experts who understood the nature of infectious diseases better than anybody.

Our history has taught us that the government is not going to protect us; we protect us. While ACT UP continues our fight to end AIDS, we also continue to fight for COVID prevention and treatment services, like free PCR tests, KN95+ masks, vaccines, and antivirals.

ACT UP New York continues "For decades, the city has operated free sexual health clinics across the city [...]. The problem of clinic overcrowding is so bad that clinics can start turning walk-ins away earlier than [their 3:30pm closing time] because the line is too long and they would never be seen by close.

 Perhaps the most diabolical aspect of the 3:30pm closing time is that clinics close right as high schools are ending their day, so students have little chance of walking in at a clinic outside of their school hours. Under 18 young people are a sexually active, high risk population that we want to reach with counseling and treatment, and DOHMH has essentially forgotten them!"

ACT UP New York answers "We are tired of the reduced hours, early closures and reduced geographic coverage at the Sexual Health Clinics. Eight million people cannot share 6 clinics. We are tired of the wind down of free COVID/Flu/RSV PCR testing and treatment services while the Flu and RSV still spread seasonally and while COVID still spreads year-round. 

We are tired of DOHMH pitting one type of care against another and asking its constituents to choose who is able to receive care and for what ailment. 

Revoking any service that once existed while that virus is still spreading is segregating the option to be treated away from those who have less money [...]."

ACT UP New York answers "The NYC free condom program gives away millions of free safer sex products like condoms and lubricant at 1,000 locations throughout the 5 boroughs of New York City. The program is managed by the city’s Department of Health and Mental Hygiene (DOHMH).

The program ships safer sex products to bars, shops, community centers, health centers, and churches which enroll for the program through a web portal. The web portal allows everyday NY-ers to search for condoms, lubricant, etc by zip code."

ACT UP New York answers "In 2023, after businesses had reopened and were seeing more patrons, we started hearing from bar staff that the condom program had not resumed and that the web portal used to order more safer sex products was broken.

We essentially pleaded with [DOHMH] how important condoms are to the LGBTQ+ community that is most at-risk for HIV, MPOX and other STDs, especially low-income people without regular access to healthcare. It was a bizarre dynamic for us as community members to have to explain to DOHMH staff how much the free condom program means to the community and HIV prevention."

New study shows increased risk of infections after COVID, COVID still mostly at “very low” levels, new drug approved for COVID post exposure prophylaxis, international disease outbreaks underscore need for global partnerships, and actions to object to MAHA.

A post-exposure prophylaxis is an emergency treatment used to prevent or mitigate infection after a potential exposure. The medication has previously been used in Japan. A recent study published in NEJM showed people in the ensitrelvir group experienced a 67% reduction in risk of getting COVID-19 compared to people in the placebo group.

The FDA’s Vaccine and Related Biological Products Advisory Committee (VRBPAC) voted to recommend a monovalent XFG vaccine for COVID-19 shots for the 2026-2027 season."

 Rotavirus is an easily transmissible virus that can cause severe diarrhea and consequential dehydration in infants and children. The current rotavirus vaccine prevents 40,000 to 50,000 hospitalizations annually. 

Rotavirus is one of the vaccines that MAHA is making efforts to remove from the current vaccine schedule. Recently, Trump issued an executive order for HHS to move to adopt the limited schedule, putting more children at risk."

Body text: "The United States has now confirmed 5 cases of New World Screwworm (NWS), a flesh-eating parasite well-known among ranchers to devastate livestock. 

The confirmed cases are in 3 cattle and 1 goat in Texas, and 1 dog in New Mexico. NWS can infect humans in rare cases. The major threat posed to humans by NWS is more about threatening the meat product supply, and therefore prices amid an already agonizingly expensive economy. 

Unsurprisingly, the US had a long-standing program to monitor, prevent, and treat cases of NWS in animals, but Trump’s DOGE completely dismantled it."

Body text: "The Ebola outbreak caused by the Bundibugyo virus continues to evolve in the Democratic Republic of the Congo and Uganda. Reports show varying levels of cases and deaths, with at least 600 cases and over 100 deaths across both countries.

Miao Cai and Ziyad Al-Aly, looking at patient data in the US Department of Veterans Affairs, compared those who had a confirmed case of COVID-19 and those who did not, among both hospitalized and non-hospitalized patients. They found those who had a confirmed COVID-19 case were at significantly higher risk of contracting another infection (bacterial, viral, or fungal) later on compared to those that did not have a confirmed case of COVID-19.

Whose lives will be changed by the freedom of choice provided by the mass availability of masks?

With the introduction of condoms, over time there were enormous changes to how groups such as women, queer and trans people, and teens were able to interact with the world and what kinds of lives they had access to pursue. Condoms gave these groups access to preventing pregnancy and STIs confidentially and without the approval of any doctors or politicians. A new sense of bodily autonomy was unlocked." The COVID pandemic has and will continue to shape all of our lives in many ways. What will our world look like when KN95 and N95 masks have been around for 30 years?

Body text: "A paper published on May 13, 2026 in the New England Journal of Medicine shows that ensitrelvir, when administered to household contacts of someone with COVID-19 within 72 hours of exposure significantly reduced additional cases. 

Given that no antiviral agents have been approved for postexposure prophylaxis, this study highlights additional tools that could be added to a layered approach to reduce COVID-19 transmission. "

Body text: "According to WastewaterSCAN, norovirus and rotavirus are at “medium” and “high” levels, respectively. 

It is important to maintain hand hygiene to protect against these infectious diseases, as they are spread primarily through contaminated surfaces, contaminated food, or contaminated water. 

Norovirus can also be aerosolized from vomit, so it is important to wear a mask if you’re caring for a sick friend or family member with norovirus."

Body text: "Measles continues to spread in the US. 

The total number of new measles cases per week has begun to taper off from the sharp incline we have been experiencing since November 2025. 

Since our last report, cases have increased from 1,877 nationally to 1,963, with 27 new outbreaks in 2026. 

In 2025, there were 2,207 confirmed cases from 47 outbreaks, an increase from the 16 outbreaks and 285 cases reported in 2024."
