Decision records (ADRs, RFCs, PR approvals): written by choice, at human cadence, and inert to the next agent run. Requirement files do reach the run, but store the rule, not who decided the exception, why, or under what condition. On the missing unit, building on Nathan Curtis's spec-driven work.

We keep asking whether a human is in the loop. Wrong question. The better one: can the human still do the thing the loop was built to let them do? Judgment isn’t something you have. It’s something you do. And it fades one unexercised decision at a time. New piece: sl1nk.com/y8x8kum

The perceived cost of delay is higher than breaking a design system constraint. So decisions keep happening. The system just doesn’t remember them.

I have family in Iran, so I think a lot about how technology shapes real-world decisions. “Human in the loop” sounds reassuring. But when systems move at machine speed, humans can quietly become observers. The real question: can they still say no?
