Prof of clin psych, Royal Holloway. Interested in how to measure what matters, especially within the CBT model. Against: red herrings, tail-wagged dogs, new wine in old bottles, straw men, reinventing the wheel. In favor of putting money where mouth is

Curious about just bashing libido and irritability in there. The whole case for the PHQ is DSM alignment, for better or for worse. The prima facie case for each item seems flimsy on different grounds--i.e., irritability more GAD or bipolar and libido just subsidiary to loss of pleasure.

Similar to the joke about all the fancy shmancy statistical techniques just being dressed up regression, anyone proposing a new all encompassing theory of psychological problems should need to show they are not just reinventing functional analysis yet again. Cf. "Process based therapy"

Scales I have developed are belief scales with either 5- or 7-point Likert scales from Completely disagree to Completely agree with a midpoint of "Neither agree or disagree". I'd suggest there is a strong argument that this tracks how beliefs are subjectively judged.

Gary Brown @paperbag1.bsky.social · 03/07/2026
This brings up another critical dimension, which is what is the scale measuring. The DERS is probably a prime example of when you could safely take out the midpoint because it is meant to measure a monotonic increasing dimension.

It's more a general point that a response category itself might not provide information, but it might be an essential factor in how responses are formed. I have noticed the neutral point is often submerged like that, but that could very well be because it is a springboard for other responses

Think aloud protocols, cognitive interviewing, also I think some techniques like IRTree can model this, but that would be something you would know more about than me.

But it could be the middle category is a necessary anchor for the one above and without it, the one above would not be distinguishable from the one above that one.

Gary Brown @paperbag1.bsky.social · 30/04/2026
Exactly. It's a non sequitur. But the "movement" needed a narrative that was more pithy than the more plausible nuanced account by Rosner (based, by the way, on interviews with Beck himself and access to his papers and letters).

I never understood the narrative line. He ends up effectively corroborating what psychoanalysis would predict but it is later spoken about as a point of inflection that led him to look at cognitions.

Yeah, but go back in time to the '90s and find Rosen with a vendetta against EMDR, denying that it could possibly work. If we want to start doxxing self mythologizing founding stories, there will be no end to it.
