Well, if it’s 6 October, it’s the 83rd anniversary of the day the world was deprived of Ignatius Trebitsch-Lincoln, who was born a Hungarian Jew, elected to the House of Commons, spied for Germany in both world wars, and eventually declared himself the 14th Dalai Lama

The "health freedom” movement is run by health authoritarians. They want to impose their anti-vaccine agenda on you, to threaten and intimidate scientists, and to let parents treat their children as disposable objects. This needs to be opposed, not appeased. My latest.

I've been loving running on the rail trail this summer and I'm dreading going back to the treadmill. I used to run on the road in the winter in my early 40s but yuck. I'm considering going back to winter running but the road camber is a killer (as are the cars). I'm hoping the rail trail is passable

And the choice of ARBs is interesting, given the bit of disparity between pharmacokinetics and outcomes data. We all have favorite ARBs out of habit, and it's unclear if there is a "Best" one.

I left out tgs but they need to be measured. It’s a good panel yours. The apo B is useful to look for residual risk esp in treated patients.

PalMD @palmd.bsky.social · 13/09/2026
different labs report things a little differently on a standard cholesterol panel. 1 of the labs i use reports nonHDL (saving me a brief subtraction in my head). Under latest guidelines, HDL and ratio are less important and LDL is emphasized more. ApoB can help sort out residual risk.

PalMD @palmd.bsky.social · 13/09/2026
It's time to redesign the standard cholesterol panel. stop reporting ratios and vldl. put in the non-HDL and apoB.

I didn’t give you all that much info but the key point here is that the patient is in hemorrhagic shock despite his normal hemoglobin. Hb and plasma are both being lost. The Hb doesn’t drop until the pt gets IV fluids.

Trick question--patient arrives in ED with altered consciousness and hypotension, blood visible in underwear on exam, tachycardic, Hb=13.8. Whats most likely diagnosis.

So it's a sort of 'trimodal' thing--tech ignorant, tech figure-it-out, tech comfortable, and tech take-it-for-granted. Is this a thing?

When I was an intern, you couldn't just call IT and get a quick fix like you basically can now. But now you often can get quick help from a bot, an It person, an AI. You don't have to feel your way thru it.

PalMD @palmd.bsky.social · 01/09/2026
They don't seem to know or care how it works and can't fix anything. that's not a criticism. They treat computers and phones like we treat old style TVs etc--best left to professionals cuz it's basically Clarkian magic.

And then people a bit younger than me were real digital natives and had an intuitive sense of how to use tech. None of them ever needed my help. But now we have my daughters generation, 2nd gen digital natives, so to speak.

If someones printer or PC didn't work right, or someone couldn't figure out how to use a program, or someone couldn't get the computer at the hospital to work, I'd fix it. My mom would say, "go slow and tell me what you're doing" but i couldn't, i just felt my way thru it.

PalMD @palmd.bsky.social · 01/09/2026
I'm noticing a trend in my own life, don't know if others see anything similar. It's about how we relate to tech. Im of an age where I had an Apple II, eventually the first Mac, always learned to troubleshoot PCs, etc. I was always the non-professional "tech person".
