Dear Secretary of State,
Re: updating public guidance on protecting health during wildfire smoke and air pollution
At 19:00 on 14th August, the Government issued its first-ever national Emergency Alert for wildfire risk, sent to millions of phones across England and Wales, following a summer of exceptional heat and a series of major fires, including the blaze in Stourbridge that destroyed nineteen homes.  We write as that alert reaches the public to raise a gap in the guidance that sits behind it — one that matters directly to people at heightened clinical risk, the community we represent.
The gap
UK guidance in this area has not kept pace with the evidence or with advice now being given by other public health bodies internationally. UKHSA’s own guidance on coping with wildfire smoke, on a blog published 14th July 2026, correctly advises that “face masks such as surgical masks are not usually recommended as they do not necessarily protect against all the pollutants that may be present in the smoke.”   But the guidance simply stops there. It does not go on to say what actually protects against those pollutants, and it makes no mention of the single intervention with the strongest evidence behind it: filtering the air itself.
By contrast, the US Centers for Disease Control and Prevention advises people to “choose a room you can close off from outside air” and “set up a portable air cleaner or a filter to keep the air in this room clean,”   and the US Environmental Protection Agency states that portable air cleaners “can reduce PM2.5 infiltration rates considerably” during smoke events.  The World Health Organization’s own wildfire smoke guidance recommends an FFP2 or N95 respirator specifically for at-risk people who must go outside  — providing helpful information on protections that the UK guidance does not.
The UK’s Daily Air Quality Index guidance, which governs advice for adults and children with lung or heart conditions and for older people on days of high and v…This leaves an important gap in the advice. Clean indoor air, achieved through ventilation and filtration, was recognised by the UK Covid-19 Inquiry as a core defence against airborne infection. There is no reason the same principle should be missing from guidance on smoke and pollution, where the underlying physics of particulate and gaseous exposure is the same.
We call on the Government to update public guidance so that it:
1.	Names indoor air filtration as a recommended measure during wildfire smoke and high air pollution, specifying HEPA filtration for particulates in line with CDC and EPA guidance,3,4 noting that activated carbon filtration can reduce gases and volatile organic compounds. 

2.	Completes the advice on respiratory protection, rather than only focusing on what is not recommended — specifying that close-fitting FFP2 or FFP3 respirator masks can provide protection for anyone who must go outside during smoke or high pollution, in line with equivalent US and WHO guidance.3, 5

3.	Expands the Clinically Vulnerable groups to reflect international standards. UKHSA’s wildfire guidance omits categories that US EPA  and European Environment Agency  guidance has previously identified as being at increased risk, such as diabetes, obesity, and populations facing higher exposure through outdoor work, income, or ethnicity. We believe UK guidance should also recognise these groups, enabling them to better understand and manage their risks.

4.	Consolidates this advice into a single, clearly signposted public guidance page, equivalent to the checklists already published by CDC and EPA, rather than leaving it distributed across separate blog posts and index pages.

This does not require new research. The evidence is available, and other public health agencies are already giving more detailed advice. We are asking the UK to reflect that evidence in its guidance.

Today we have written to the Health and Environment Secretaries, backed by 20 experts. UK guidance says shut the windows. But research found smoke in all 23 Manchester homes monitored during July's fires. Don't just tell people to stay inside. Tell them how to stay safe. 1/2

New DfE schools guidance on clean air - good news for kids who are clinically vulnerable, or have asthma or hay fever - and everyone else. Air cleaning (ventilation or HEPA filtering) removes pollen, mold spores, particulate matter (PM) air pollution, and respiratory droplets that spread diseases.

Six weeks. One in five infected may die. The public deserves to see the plan.

Clinically Vulnerable Families (CVF), a participant in the UK Covid-19 Inquiry, today call on Health Secretary Wes Streeting to immediately publish the full operational and clinical arrangements for passengers and close contacts linked to the MV Hondius hantavirus outbreak.

Exposed passengers are expected to leave supervised assessment today and continue isolation at home for around six weeks.

CVF fully supports the need to protect the mental health and wellbeing of those affected, and no one should remain in supervised isolation longer than necessary. But if people are being asked to isolate at home for exposure to such an extremely high-risk virus in which at least one in five infected people die, the public deserves to see the plan.

UKHSA has said that "robust arrangements are in place." Reassurance requires transparency.

What happens over the next six weeks will determine whether this outbreak might fizzle out quietly or it has the potential to becomes the UK's next health inquiry.

Whilst this is not considered a pandemic virus, this moment is reminiscent of the early days of the Covid-19 pandemic, when critical decisions were being made behind closed doors and the public was asked to rely on assurances rather than a precautionary approach relying on known potential risks and transparency over operational details.

CVF is calling for the immediate publication of the full home isolation framework for passengers, including infection prevention advice, respiratory protection, testing arrangements, transport protocols, monitoring, welfare support, and the measures being taken to protect affected households, healthcare workers, and anyone who may come into contact with isolating individuals, including delivery drivers, household members, carers and emergency responders.

important that the Government publishes the full plan so affected families, healthcare workers and the public understand exactly what measures are in place to protect everyone who may be exposed in the course of ordinary life and work.

A recent opinion article in the BMJ by Trisha Greenhalgh and colleagues argues that this outbreak should reset the world's default approach to airborne risk and precautionary infection control.

As a Core Participant in the UK Covid-19 Inquiry, CVF successfully argued for recognition of airborne transmission risks and the importance of close-fitting respirators such as FFP3 masks in healthcare settings for staff and patients when there are airborne risks.

Statement from Clinically Vulnerable Families:

"Passengers are about to leave hospital for home. Mr Streeting may be about to leave office. Whichever happens first, the plan must be published first. This is not seen as a pandemic virus - but it is a high consequence one. We are asking that the families, NHS staff, neighbours and delivery drivers who will encounter these passengers over the next six weeks can see the protections in place. If this is your last act as Health Secretary, make it count."

This tragedy was not unforeseeable. Dr Christine Peters, a consultant microbiologist, repeatedly warned that conditions at the hospital were unsafe - especially for immunocompromised patients. Her warnings were ignored. 2/2 news.sky.com/story/it-cos...
