Slept through my telehealth psych appointment cause I’m on rotating night shifts. Fortunately was able to get another slot for later today. Also got dinged for a $100 no show fee which is not great on resident salary. We’re having a good time here

How do I know Gabapentin works for neuropathic pain? Probably the 9000 calls I get overnight for older patients taken off their home Gabapentin for concern for altered mental status / delirium and now they’re complaining of burning tinglies and electric ants crawling on their feet

Once saw a patient with Huntington’s who was able to remain in home longer + preserve relationships because low dose antipsychotic made him less explosive. Off label, sure. But you know we use these meds to treat hiccups and vomiting as well

Patients with Alzheimer’s have a terminal illness that is predictably destroying a series of anatomical structures in their brain. Attempting to treat the known psychiatric manifestations of this process, weighing risks and benefits, is not at all the same thing in principle as a chemical restraint

Lots of vapid buzzwords: “brain-altering meds” (so is Tylenol). “A clash over whether to drug” (give care that is indicated). And so on. Do we still have editors

I understand the guilt that families have when they can no longer care for loved ones. I also recognize that psychotropics can genuinely be abused to sedate rather than to treat symptoms. But we need to be honest about what dementia is, and not ignore genuine suffering because of fear or guilt

When my grandmother’s dementia progressed she was often in a constant state of panic, feeling she was in a wrong & dangerous place. She was in a nice nursing home with adequate staffing. Lots of redirection & behavioral interventions like dolls & towels to fold. Still needed meds, SSRI and prn benzo

Agreed—my dad has dementia and we went through a period last year where he was attacking caregivers, hyperventilating, and incredibly anxious all the time. It was horrible for him. Getting him on antipsychotics was a difficult choice, but it’s been one of the best descisions we’ve made.

Another issue raised in this article is how to deal with pyuria / bacteriuria in nursing home residents. My understanding is that without localizing symptoms or objective signs of infection, should not jump to abx. But I’ve seen a lot of variance in practice

Anyway the actual answer is to massively increase public spending on elder care so that facilities are staffed and resourced well enough that all of the effective behavioral interventions can be implemented wherever they are needed

I believe whether or not meds constitute chemical restraint from the perspective of the joint commission depends on the purpose for which they are given. If only to restrict movement it’s restraint. If given to improve agitation & alleviate distress, & reversible causes addressed, it is treatment.

Articles like this often feel really out of touch. Of course behavioral interventions should be tried first, and this is often limited by staffing.
