A case of mechanical removal of IE vegetation - fascinating. I wonder if this really "removes" the infection from the valve. But maybe resulting less bacteriemia and septic embolisms are helpful by itself. #cardiosky

This is interesting. For years we assumed that tachycardia/arrhythmia causes EF drops. Now it seems, in some people afib may just be the start into a HF career. Not sure, though genetic testing big scale is feasible. Guess we just have to keep them on a short leash. www.jacc.org/doi/10.1016/...

In my department we still fullheartedly recommend open heart surgery for <70 yo with aortic valve disease just like the updated ESC say. Increased re-intervention rates in TAVI patients strengthen that position.

A HF patient has an LVEDD of 400 ml is treated with OMT and shows a huge functional MR on Echo. EROA 0,4 - would you consider this patient for M-TEER? (QRS is narrow so CRT not an option). I have an opinion but curious what others think. #cardiosky

At a congress recently someone said a stress echo request causes stress in every cardiology department. Not in ours. The patient is put on a bike and we make beautiful images like these here. Even (or: especially) with LBBB. #echosky

Nice slide from @alexmla.bsky.social‘s great presentation at the 3D- #echosky workshop in Berlin. How to visualize the MV scallops by just pushing and pulling the probe. Memorize: push 1➡️2➡️3. Personally I prefer biplane which yields similar results but not everybody may have 3D-TOE.

Levosimendan is quite often seen as a "safe" inodilator with respect to ventricular arrhythmias. I don't think it is, and this trial in my view supports that. Also, drugs with very long half lives are generally not a good idea in the critically ill #cardiosky #emimcc #cccsky

❓Why do speckle tracking strain algorithms measure end-systolic strain instead of peak strain? While peak strain values might be similar, the timing can vary significantly in ischemic myocardium. Using aortic valve closure as the reference point is the only way to clearly reveal the pathology.
