did an intervention today in SAO (Starboard Anterior Oblique) but an unprotected Port Main intervention should only be done under orders of the Admiralty unless in danger of being Sunk, Burned, or Destroyed

yeah, that's great until you have two other (more) critical medicines that need to each be taken separately on an empty stomach I'd have to get up at 4 am to start taking pills I'll just live with a ferritin of 23, that's the best I can do (and no one seems to want to give me IV)

well, I agree, but we also have host-exam, and stent short-dapt trials (xience 28/90, onyx one-clear) etc the problem is in the real world the person making the decision isn't routinely the one who did the procedure, nor at the time of the decision do they review the presentation and report/films

I don't think the MR studies are misleading, nor are the studies regardless of genetics that show residual risk the question is only is if this is the right rx strat call me in a year or two when we have more robust results

I await the paper to see the PCSK9i use rates between groups as that may have biased to the null
110
Josh Krasnow @jmksnow.bsky.social · 06/09/2026
IDK about that. There is data to suggest residual risk (regardless of genetics) at increasing Lp(a) levels. There was no data on Lp(a) lowering because there was no *pure* agent to do that but yes, this suggests Lp(a) lowering adds little additional risk reduction on top of aggressive standard rx

I like to use the airport screening analogy: it's a pain in the ass, and when your plane isn't hijacked, all you experience is the downside. You don't appreciate that you arrived safely at your destination (in before the "security theater" comments)
010
Josh Krasnow @jmksnow.bsky.social · 06/09/2026
yuh, I know about the study. It's not about the genetics, or if Lp(a) conveys excess risk. It's if specific Lp(a) lowering with this agent conveys additional risk reduction on top of good medical therapy. Don't understand your question

Josh Krasnow @jmksnow.bsky.social · 06/09/2026
I think pretty well treated groups, so hard to show a difference with very low # of events. Also PCSK9i's used as well which bias to the null (20-25% Lp(a) reduction) full paper and upcoming competitors will be interesting

The town of Phil Campbell, AL was named after...Phil Campbell... who built the railroad depot Every year they have a festival at which Phil Campbells from all over come to celebrate the non-uniqueness of their name

I would add OpenStreetMap, which is also community edited. This administration has really underscored some strengths of the wiki model.

Although I don’t moderate this section of the forum, here’s a friendly reminder from last time the US government instigated a frivolous naming dispute over an international body of water:

These are the guidelines we use to determine what should be considered the name of a feature:

Names
Multilingual names
Disputes
Currently, the Gulf is tagged with name=Golfo de México, name:en=Gulf of Mexico, and official_name:en-US=Gulf of America. This is unlikely to change, unless the US government reverses its position, or (less likely) English speakers across the world all agree to start using the US government name. That last tag, official_name:en-US=*, is quite obscure and doesn’t show up on any map renderers that I’m aware of. But its inclusion allows people to, for example, search for “Gulf of America” and be redirected to the Gulf of Mexico.

Please keep comments on-topic and substantial. We all have opinions, and it’s easy to drop your two cents in an online forum. However, a discussion about OSM naming guidelines or real-world name usage statistics might be worth at least a few dollars here.

In this case, the lake is tagged with name=Lake Ontario, official_name:en-US=Lake America. For this situation to change, the government would need to reverse its position.

The guidelines on naming disputes apply to every country equally. This is far from the only naming dispute around the world. Let’s not get bogged down in American exceptionalism when we consider how the tagging guidelines should or shouldn’t change.
