Our world is crazy and I want to continue fighting evil things, but today I have a book release that I'm really excited about and so I want to also have just one day to be really happy about that.

(*not medical advice* disclaimer) Generally, the upper limit cut-off for HPV vaccination is age 45, and that should be a risk/benefit discussion w/ your doctor. I'm very glad that your daughter got her HPV series. So smart!

I feel like there must have been a similar cheer when the results of phase 1 trials of mRNA vaccine for pancreatic cancer were released. I know I cheered! I'm holding my breath in hope for the expanded trial results.

HPV vaccine to prevent head/neck cancer is a big deal. I have dx'd two gnarly head/neck cancers in pts both male and in their 40's. HPV cancer. Could have been prevented (actually probably not for them, bc they were too old for vaccine at the time - this was many years ago) HPV cancer: preventable

It's such a weird thing on my day off, in the middle of editing a MS, get a call from hospital about an urgent patient issue, log into the EMR to place orders, call specialists to set up consultations, call patient, then just...go back to editing? No wonder why my creative brain gives up at times.

100% true. It is beyond confusing to switch from Medicare Advantage to regular Medicare + supplement. Nearly impossible. It's like you're trapped in a prison.

Thanks much. I shout this to my patients all year long and then pop a thread into social media every fall. Most folks don't understand the Medicare Advantage scam. If I can reach a few extra folks here and there, it feels like I'm doing something worthwhile to make people's health a little better.

I also know that denying a 66 y/o cancer patient who can't get the oncologist-recommended cancer treatment because the less-effective drug is cheaper, all while the CEO of your company makes $50 million per year ... ought to send your company straight to hell for killing people out of greed. 16

What I know is that if you have to pay $1000/month for insurance and you have a $15,000 deductible, then you've paid out of pocket $27,000 to the company BEFORE THEY GIVE YOU ANYTHING. per year. January 1st? Rinse and repeat. Every. Flipping. Year. 15

Listen, entire hospital depts exist because of the stupidity of coding, billing, prior auths, claim denials, resubmissions of same exact stuff, claims adjustments. Some depts are like 20 employees deep including managers. All bc insurance companies exist to make things cheap, but not healthy. 14

The creep of Medicare Advantage becoming a parasitic infection in the Medicare market is pretty awful from a patient care perspective. It's hard to watch as a doctor. I just keep trying my best to educate patients and keep up with the stupid Medicare and MA changes, but ... 11

A Medicare supplement covers that $10,000 gap. Yes, you pay for the coverage per month. Caveat: sometimes folks can have that supplemental or "gap" covered under state Medicaid if they are low-income or disabled. (depends partly on the state and how poor and how disabled as to who gets it) 10

You get a lab test to recheck your thyroid level, and the lab test costs $200. If you don't have a supplemental, then you will owe $40. Cool, cool, cool. If you are in the hospital with pneumonia for a week and rack up a $50,000 bill without supplemental coverage, you owe: $10,000... Oh. 9

You DO know that basic Medicare (or MA for that matter) DOESN'T COVER EVERYTHING, right? Depending on a lot of factors. With Medicare, for example, if a test is screening vs diagnostic, it changes the coverage %. Screening = 100% covered. Diagnostic = 80% covered Let's do some math, shall we? 8

Instead, get: Medicare part A (hospital coverage) Medicare part B (doctors visits and home health care and equipment like walkers and wheelchairs) Part D (drug plan) Part G/K or "Medicare supplement" (what the above doesn't cover) WHICH REMINDS ME TO MENTION... 7

What to do? (again, do not construe as expert advice) If you are on a Medicare Advantage plan, try to switch. Good luck. They make it stupidly hard to do so, but it's possible. Reminder, they care only about your money, not your wellbeing. They will do anything for the former but not the latter 6

Some of the most awful human sacrifices I've seen by insurance companies have come from MA advantage plans withholding lifesaving treatments for patients. Heck, if they delay long enough, the patient dies and they get to keep the Medicare money they refused to spend on the patient. Yay! 3

MA plans are BASICALLY SHITTY PRIVATE INSURANCE. Here's how it works. You get XX amount of money from Medicare each year for care. MA plans take your Medicare money and then turns around and tries not to spend it. That's right, they make money by not spending *checks notes* YOUR MEDICARE MONEY 2
