Sunsets, disasters, dinners: we now record them all with our phones. But what is that doing to our souls? | Phoebe Greenwood
As a war correspondent, I worked with photographers who said the camera was a buffer between them and reality. It is much the same now as we snap and scroll, says journalist and author Phoebe Greenwoo...

Also by the way it's late where I am, I'm quite happy to come back to this tomorrow, but I need to get some sleep.

I think there are very few 'well established facts' about the modern incarnation of gender dysphoria. What we appear to have is a social contagion driven by teenagers and children, not of whom are old enough to realise or understand that this is a new thing. And sadly, they are being let down by ..

And if there's no possibility of a gold standard test, if we're relying entirely on patient self-reporting, well that does I'm afraid suggest something in the head not the body. It's suggests either a psychiatric problem, or at least a social one. But not a medical one worthy of medical intervention

and why are you using the DSM to justify your arguments? That would suggest at least you believe that being trans is in fact a psychiatric condition. If it's in the DSM, likewise, it suggests a psychiatric condition, not a 'medical' one, so why are we treating it with medical drugs and surgery? ...

But non-psychiatric conditions are rooted in bodily anatomy and physiology, invariably leading to the possibility of gold standard tests. So if it's not psychiatric, where's the gold standard test ...

But it *is* a psychiatric condition. So here's one problem I see, you're right that there's little in the way of gold standards in psychiatry, and that's just one reason some doctors and clinicians don't see psychiatry as real medicine. 'Gender medicine' claims it is real medicine, and many 'trans'

JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Right, finally, something concrete. Persistent incongruence, desires about sex traits, desires about social treatment. You've already used the word 'social' so I'll get straight to it: someone presents with all 3, how does the doctor differentiate between medical condition and social contagion?
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
You scatter around an awful lot of generalised top down links for someone seemingly so sure they're right.

JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
If you have no gold standard test you can't calculate sensitivity and specificity for any other proposed test, you're literally working blind when trying to assess the usefulness of diagnostic methods.

JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
More importantly, what should a doctor look out for in order to spot gender dysphoria before the patient mentions it? Because that should be possible, right? And gold standard tests are vital by the way, because they're used as the standard by which other, up coming diagnostic tests can be assessed.
200
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
So what are those signs of gender dysphoria. How can a doctor spot gender dysphoria in a patient?
200
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Anti-depressives by the way, are relatively straightforward, don't cause life-long mental impairment if used correctly and for short periods and, critically, don't involve the patient lopping body parts off. Gender dysphoria however is not as clear cut. I'm asking you what the signs are.
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Depressives also tend to speak in a monotone, hunch or slouch their shoulders forward and cast their eyes downwards. Most GPs can make a fairly good diagnosis of depression before the patient ever mentions it. It's fairly, usually time limited, and usually responds to anti-depressives.

No, a transwoman is a man. By definition. You have to be a man in the first place to make the claim. I for example never need to claim to be a woman, I just am one and everyone can see that.
110
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Yes but what are the signs? All you're telling me is that diagnosis is based entirely or almost entirely on self reporting, which is very dangerous. And you seem to be suggesting there is, in fact, no gold standard test. So, how can we tell when a patient is lying about their condition?
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
No I didn't say that and I've just dealt with it, see my other post.
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
And just another aside, but a diagnosis of depression, for example, is well supported by signs, and there are some really interesting ones. We can identify altered brain chemistry in a depressed person; a depressed person has an enhanced startle reflex; people with depression don't self care.
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
So it's quite factually correct and not at all baseless. I don't care what your sense of yourself is. I don't care if you identify as an egg plant, have at it mate. And the documented science behind gender well-being is cherry picked at best, frequently not followed up or verified by long term study
100
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Also, just an aside, but the DSM 5 is controversial because it imposed a switch from diagnosable full conditions, linking drug prescription to formal diagnosis, to individual manifestations of symptoms, linking instead the drug to the individual thing it alleviates. There are pros and cons.

the signs of so called 'gender dysphoria'? By signs I mean observable medical indications, so i.e. not symptoms, symptoms are what the patient reports. I'm not asking about what the patient says, I'm asking what are the clinical signs, what is the gold standard clinical test?

I have a question for you. What is the current gold standard test for so called 'gender dysphoria'? Is it visible on a scan? Is there a biomarker test? If we assume it's a genuine medical condition then there must have criteria for diagnosis, signs in other words, backed up by a test. What is it?
110
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
Ah yes, the one and only exception, CAIS. It's very unfortunate. It also doesn't mean that all the men claiming to be trans are actually women.

JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
If your sex chromosomes include the XY combination plus an SRY gene you're a man, otherwise, a woman. I really don't care how you identify, identify how you fucking like, but you aren't redefining the words 'man' and 'woman'.
110
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
And while you may like to define sex as a 'cluster' the reality is it's the basis for reproduction therefore comprises two and only two forms (or as a species we'd be constrained to having sex in clusters instead of pairs 🤨) and while there are variations the Y and SRY gene are pretty definitive.
110
JazzTulip (Louise) @jazztulip.bsky.social · 16/07/2026
You've defined man and woman as gender categories because you're working within the made up construct of modern gender ideology that seeks to deconstruct existing personal and social boundaries, something which is very convenient for sexual predators. I don't believe in your new-definition.

We're going to have to disagree on language then aren't we? 'Male' is the adjective describing 'man' (XY sex chromosomes), 'female' the corresponding adjective for 'woman' (XX sex chromosomes). We know you want to redefine language. We're not buying it. Science + I stand by want I wrote.

Oh, and by the way, I think that paper you linked agrees with me. It is very clear that there is a difference between sex and ‘gender identity’. Which is pretty much what I said in my post. You can claim the gender identity of transwoman, meaning you are of the male sex, i.e. you are a man.

This is not an intolerant post btw, it is a factual post, a scientifically factual post. Now you can either accept science, biology and reality, or you can stick your fingers in your ears and chant “TWAW” over and over until you faint through lack of oxygen. Your choice. You won’t change reality.
