Key questions remain: is virus still replicating, where is it, can the drug reach that tissue, and is the virus still sensitive to that drug? Main paper: doi.org/10.1038/s414...

That does not mean antivirals cannot work. It means persistence trials may need resistance monitoring, combination strategies and drugs that hit different viral targets.

For Long COVID, the relevance is viral persistence. If some patients have actively replicating virus in tissue, repeated or prolonged single-class antiviral treatment could select resistant virus.

Jack - amatica health @jackamatica.bsky.social · 22/07/2026
This is why antiviral resistance matters in chronic viral diseases like HIV. HIV treatment usually uses drug combinations, partly because one pressure point can let resistant virus take over.

Jack - amatica health @jackamatica.bsky.social · 22/07/2026
Cross-resistance means resistance to one drug may also reduce sensitivity to another drug, especially when both drugs hit the same viral target.

The resistance patterns also affected nirmatrelvir and ensitrelvir. The study highlighted mutations including S144A, E166A and E166V, with E166V showing the broadest effects.

Testing showed strong cross-resistance among viruses selected against atilotrelvir, ibuzatrelvir and simnotrelvir.

Jack - amatica health @jackamatica.bsky.social · 22/07/2026
Resistance appeared through overlapping mutation pathways. That means the virus could use some similar escape routes against several drugs in the same class.

Jack - amatica health @jackamatica.bsky.social · 22/07/2026
Researchers grew virus under drug pressure to see which escape mutations appeared. In plain language, they watched how the virus changed when forced to survive around antivirals.

The study focused on 3CL protease inhibitors. These are drugs designed to block that viral enzyme. The class includes nirmatrelvir and ensitrelvir, plus newer experimental drugs.

Paxlovid contains nirmatrelvir, which blocks the SARS-CoV-2 3CL protease. That protease is an enzyme the virus needs in order to copy itself.

A new study found overlapping resistance patterns across Paxlovid-like SARS-CoV-2 antivirals. This matters for future Long COVID viral-persistence trials, where resistance could block clearance, similar to why HIV treatment uses combinations. Let's break it down.

The study was small and preliminary. It cannot prove that reduced glymphatic function causes ME/CFS symptoms or that changing glymphatic flow would improve the illness.

This fits a real ME/CFS question: could disrupted sleep and brain clearance biology be interacting with cognitive symptoms?
110
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
Plain meaning: the people with stronger sleep and concentration problems tended to have a lower MRI signal related to brain fluid movement and waste-clearance biology.
100
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
Lower global DTI-ALPS was linked with worse sleep disturbance and worse impaired concentration scores.
110
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The right-hemisphere DTI-ALPS index was also lower in ME/CFS, while the left side was not clearly different from controls.
110
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The global DTI-ALPS index was lower in ME/CFS than in controls. The reported values were 1.44 in ME/CFS versus 1.51 in controls.

Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The glymphatic system is often described as the brain's waste-clearance system. It helps move fluid through the brain and is thought to be most active during sleep.
100
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
DTI-ALPS is an MRI-based estimate of glymphatic function. It does not directly watch waste leaving the brain, but it gives a signal related to fluid movement along small brain spaces.

Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The study included 31 people with ME/CFS and 27 healthy controls. Researchers used 3T MRI and a measure called DTI-ALPS.

An interesting new MRI study found a lower brain waste-clearance signal in ME/CFS, linked with worse sleep disturbance and impaired concentration. Let's put that in simple language.

Both studies are preprints, so they need peer review and replication. Main preprints: doi.org/10.64898/202... and doi.org/10.64898/202...

Cytokines are immune messages. So the plain version is: saliva may show both viral activity signals and host immune-control signals in Long COVID.

Jack - amatica health @jackamatica.bsky.social · 21/07/2026
That study found salivary microRNA patterns linked with inflammation and viral persistence pathways. In lab oral cells, selected microRNAs changed inflammatory signaling and cytokine production.

Jack - amatica health @jackamatica.bsky.social · 21/07/2026
A second saliva preprint looked at microRNAs. These are tiny molecules that help turn gene activity up or down, almost like small regulators of which cell programs get louder or quieter.

That does not prove HHV-6 is causing symptoms. It could be contributing to symptoms, or it could be a marker that the immune system is under more stress.
100
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
Plain language interpretation: HHV-6 may not separate every Long COVID case from every control, but it may track with how severe illness is in a subgroup.
100
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The more interesting signal was severity. Higher HHV-6 DNA in saliva was linked with greater overall Long COVID symptom severity, plus higher anxiety and depression scores.
100
Jack - amatica health @jackamatica.bsky.social · 21/07/2026
The study did not simply find that Long COVID patients shed more EBV or HHV-6 overall. That part was not clearly different between groups.

They tested for herpesviruses including EBV, HSV, HCMV and HHV-6. These viruses can stay in the body after the first infection and may reactivate later, especially when immune control changes.

One preprint studied 45 people with Long COVID and 45 matched controls, using repeated saliva samples collected across two days.

Two interesting new saliva studies found HHV-6 viral DNA linked with Long COVID severity, and microRNAs, small gene-control molecules, linked with inflammation and viral-persistence pathways. Let's get into it simply.

Large RNA datasets have already become clinical tools. Tempus classifies uncertain cancers. Veracyte assesses thyroid nodules. Oncotype DX helps guide chemotherapy decisions. Inflammatix uses 29 whole-blood RNA signals to assess infection and severity.

Machine learning/AI can compare RNA with symptoms, severity, and treatment histories. Gene networks can highlight possible disease mechanisms. Scale also lets us build a model in one group, test it in a separate group, then check it again in future patients.

Jack - amatica health @jackamatica.bsky.social · 17/07/2026
The main opportunity for Amatica Health is scale. In a study of 50 people, a subgroup affecting 1% may appear once or not at all. In a database of 20,000, the same subgroup could include around 200 people, allowing much deeper analysis.

In sepsis, RNA has identified immune groups that may respond differently to steroids. In lupus, it found four molecular subgroups. Whole-blood RNA has also helped diagnose rare genetic diseases when DNA testing alone did not give a clear answer.

In childhood fever, blood RNA and machine learning/AI classified 18 infectious and inflammatory conditions. A separate whole-blood RNA study helped distinguish MIS-C from Kawasaki disease, bacterial infections, and viral infections.

Similar approaches have worked elsewhere. In tuberculosis, whole-blood RNA has been used to predict who may progress from latent infection to active disease. Other blood RNA studies have linked RNA patterns with treatment outcome.

Jack - amatica health @jackamatica.bsky.social · 17/07/2026
Earlier blood gene-expression work found seven ME/CFS subgroups with different symptoms and severity. A later single-cell study found monocyte patterns linked with severity, along with platelet RNA changes after symptom provocation.

Jack - amatica health @jackamatica.bsky.social · 17/07/2026
Prior findings in ME/CFS A whole-blood RNA study found 176 altered genes, including lower B-cell development signals and higher antiviral and inflammatory activity. The wider pattern was also linked with cortisol, heart-rate control, and PEM scores.

Protein studies have also found inflammatory and less-inflammatory Long COVID groups involving interferon signalling, neutrophils, B cells, and autoreactivity. Together, these findings support studying Long COVID as several biological groups.

Another study combined RNA, proteins, and machine learning/AI. It found Long COVID groups with higher or lower neutrophil activity and identified five RNA markers that strongly separated the higher-activity group during internal testing.
