Little old lady, 50 kg who gets an axillary for a radiusFx, 30 ml max, and then some guides call for reduction 20% 24 ml. 5 ml for intercostobrachialis. That leaves 5 ml/nerve. And the reg has already started but some of that dose may have ended up in the wrong place.

I'm looking to shift our shop to Ropivacaine single shot for regional, surgical anaesthesia. I feel the evidence for 3mg/kg max is shaky, and from a pre-ultrasound era. Using 5 mg/ml, I might hit the roof, and need more volume. Any references and thoughts appreciated. #AnSky

The sound you are hearing from the Trump administration is political panic. Because of all the corruption they cannot afford to lose the 2026 midterms, but they know in a free vote they are looking at defeat. The implication of this is that they will try to thwart a free vote.

Guidelines stipulate that a PENG block should have the same restrictions as neuraxial block with regard to anticoagulation. This strikes me as overkill, just based on anatomy. And considering the damage from a 21G cannula compared to, say the hip-fracture. Anyone have any thoughts on this?

I second that. CRP is the first thing ID asks, will make or break a discharge, and we all feel so much better if the curve finally turns down. But I remember Psidres writing on twitter that in his ICU in NZ requesting a CRP is a fireable offence.
