My experience across global health tells me we're thinking too small. We're trying too hard to improve the systems we've inherited, and not hard enough to transform them.

A timely piece from Margaret MacMillan on the changing international order and cooperation within it. It raises a question I keep coming back to. What happens to problems that depend on collective action when national and collective interests diverge? www.nytimes.com/2026/09/01/o...

If academia wants to draw a hard line around AI-assisted writing, it also has to be intellectually consistent about the human-assisted writing practices it has tolerated for decades.

There was a time I saw part of my PhD research presented as a poster, with someone else's name as first author. Plagiarism and intellectual theft are far more common in academia than academia likes to admit.

We’ve entered a post-industrial era of innovation with institutions still designed for the industrial one.

Religion was once a claim about what deserves devotion. The rich learned to turn it into a claim about what deserves obedience. Innovation was once a claim about what deserves investment. The rich have learned to turn it into a claim about what deserves escape.

AMR deaths rose around 8% between 1990 and 2021, and are projected to rise another 70% by 2050. At what point do we stop asking how to do more of the same, and start asking whether we're solving the right problem?

So the next innovation may not be another programme or another funding mechanism. It may be assigning the preservation function our governance architecture still lacks. That may be the innovation that matters most.

But they also reveal something deeper: we keep asking finance to compensate for a governance function that remains unassigned. When governance is incomplete, finance compensates. When governance is well designed, finance catalyses. Institutional design is the multiplier.

Success upstream raises the cost of leaving preservation unowned downstream. The funds, incentives and partnerships built to support antimicrobial innovation have been essential.

The blueprint also points to AI as an area of future opportunity for diagnostics and discovery. Those advances matter, yet the faster AI delivers new therapies, the greater the need for an institution responsible for keeping them effective.

Who holds the enduring mandate to preserve antifungal effectiveness itself? Diagnostics, access, surveillance, stewardship and innovation are all essential. But none of them assigns any institution responsibility for keeping antifungals effective as resistance develops over time.

It’s a shame that the 5th High-Level Ministerial on Antimicrobial Resistance has been postponed. I was hoping there would be clarity on who is actually responsible for preserving antibiotic effectiveness as a long-term strategic capability. Postponing the meeting postpones that question too.

The paper is best read downloaded rather than in the SSRN browser preview. Happy to receive critique, especially on where the framework may be under-specified or where particular governance dimensions may be doing too much work in the index.

Antibiotic effectiveness is a shared strategic capability, but its durability is determined by the weakest preservation architecture we are willing to tolerate.

🇺🇸 The United States scores Tier C, despite BARDA, CARB-X, and some of the world’s most advanced regulatory pathways. The regulatory and commercial structures that help a drug come into existence are not the same as those required to preserve its effectiveness.

Across 31 countries, ARGI shows that national capability does not reliably translate into custodianship. 🇹🇭 Thailand scores Tier B, the only low- or middle-income country in the sample to do so.

If AMR is as urgent as we say it is, are we convening to decide, or deciding to convene?

I find the psychological side of AMR fascinating. It is so hard to provide a new perspective to people working within it. Thats probably why I enjoy the challenge. Compared to other disease areas I actually get paid to operate in its definitely the hardest from a human perspective
