Virulent Y. pestis? In sub-BSL-3 conditions? No immunization, no immediate treatment? How the hell does a case like this happen?

onset -> suspicion of plague = 4 days. onset -> definitive identification of Y. pestis = 8 days. onset -> death of index patient = 2 days

I have a pre-op appointment for ankle surgery in Seattle in late October and I haven’t bought the tickets yet because I have logistics to wrangle but the tickets I was eyeing have gone up over $100 in 2 days lmfao

At some point it becomes cheaper to buy the refundable fares than to wait to clarify travel plans and risk fares going up but man do I not want to pull that band-aid off right now

Side open because we lost the cover, monitor image bowing inward (would sometimes fix itself with a good slap; dreadful on 800x600), 256MB of RAM (at least originally) glory. Ran horrifically adware-infested Windows XP that I spent years taming til I finally installed Linux

SPECIAL REPORT Last month, Alpha School invited people to follow student social media accounts "if curious about life in our high schools." I accepted their offer. What I've found shocks me. Forget the AI and ask instead: Is Alpha Schools a cult? Follow along for a summary of my findings.

There is a book kicking around somewhere in my heart about my family history w/ congenital rubella. My mother was one of the babies of the ‘64-‘65 epidemic in the US; that shaped 3+ generations of our family. One little virus has such an outsized impact. Every infection prevented is a victory.

But we grow complacent, and we forget those who continue to suffer, and we forget even our own risk. And we get caught sleeping at the wheel, and we deal with the consequences. Unless we are all protected, none of us are. Eradication must be the goal.
000
Katrina Guerrier @guerrier.bsky.social · 26/08/2026
Reintroduction is a constant threat, and the demands are high to protect against it. Surveillance, high vaccination rates, maintenance of the system that allowed for elimination in the first place. Disease anywhere is a threat everywhere. A pathogen is always a plane ride away.
110
Katrina Guerrier @guerrier.bsky.social · 26/08/2026
That apathy can actually render the true goal, eradication, more difficult. Eradication protects all people, throughout the world, for all time. Elimination protects a particular community for a time. But others remain at risk, which is unacceptable. We also remain at risk.
100
Katrina Guerrier @guerrier.bsky.social · 26/08/2026
The reason I insist on the distinction is twofold: equity & future risk. An eliminated disease remains a threat to a child, somewhere in the world, right now, where the disease remains endemic. But we are often unconscious of or apathetic towards that threat because it is Elsewhere.

Katrina Guerrier @guerrier.bsky.social · 26/08/2026
This will perhaps sound like pedantry and I apologize. I truly don’t mean it as such. To eradicate is to uproot (e-radica): to disrupt all chains of transmission everywhere in the world. What we’ve done with polio is elimination, banishing (e-limina, outside the borders): ended transmission here

new pilgrimage to make next time i'm visiting Seattle
000
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
I'm also curious if there are just more people out there logging what they eat now. Certainly when I logged all of my meals when I was tracking nutrition hard, tracing would've been easier
000
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
The incubation period for cyclosporiasis definitely contributes, too - I certainly don't remember what I was eating 2wks ago. One thing that could be leveraged for more intel there is bank statements for restaurants and grocery loyalty/discount cards since those track products bought
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
Longer time to identify & link cases means contaminated food remains available to sicken more people for longer. The lack of effective genomic epi tools for cyclospora has come up a few times & probably contributes (and that's a consequence of the nature of the pathogen)
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
One of the things I took away from this exercise wayyy back when was that cyclospora causes relatively few outbreaks but the outbreaks that it does cause are quite large, with multiple outbreaks >500 cases. As best as I can tell this is because we have such difficulty tracing it
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
But all the same, this outbreak, right now, is an order of magnitude larger in scale than any of the ones in my spreadsheet (could be due to better case-finding, could be the sheer lag time)
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
I have my suspicions about those ones, obviously. I was trying to find any info I could to link them together and possibly to link them to one of the other outbreaks with a known supplier, but I ultimately was unable to do so
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
The other outbreaks were the Del Monte vegetable tray outbreak (250 sickened, 2018) and annual summer outbreaks associated with cilantro from Mexico that were never linked to a specific farm (813 total cases)
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
Another outbreak in 2018 was linked to salad mix used by McDonalds that sickened 511; also supplied by Taylor Farms
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
I noted the products & suppliers for these. An outbreak in 2013 sickened 631 and was linked to salad mix obtained from Darden restaurants supplied by Taylor Farms
100
Katrina Guerrier @guerrier.bsky.social · 23/07/2026
fun fact: back in 2018 I went through FDA's website to catalogue every foodborne illness outbreak investigation from 2013-2018 (everything on the website at the time). i saw 6 cyclosporiasis outbreaks linked to a total of 2,205 cases

Latitude, too. When I last visited Seattle at the winter solstice they had a full hour less daylight than we did in DC at the time; in summer, a full hour more (very disorienting in the evening with DST). Hence why I lost it seeing Patty Murray excited about perma-DST!! Daylight from 9-5 in Dec!!
010
Katrina Guerrier @guerrier.bsky.social · 15/07/2026
I tend to lean towards permanent standard time for this reason but as someone who lived in Seattle for 10y perma-DST just makes me seethe. I‘d just take the dang time changes over that.

sleep/awake isn't a binary, that morning light does increase your wakefulness, and having it later in your day will make you worse

Katrina Guerrier @guerrier.bsky.social · 15/07/2026
yeah you might rise before dawn but the # of us who have to go multiple hours at work/school before dawn is small and we already know that those who do have to do that trade their health for it. now we're just gonna make everybody trade their health for it
