David J Black's articles in Scottish Legal News give some background. Also, Wessely's wife had significant influence within RCGP and is now in the House of Lords. Other BPS were in core positions globally.

Worse, echoviruses are part of the EV-B species i.e. the hub of the original ME archetype. Thanks to the CFS debacle, people (including researchers) were redirected to HHVs and 'anything but EV-Bs' (if that sounds familiar, 'anything but COVID' excuses often come from the same people/organisations).

Much will depend on what individual cases have. We need a range of treatments and we need to be particularly careful when choosing them. Treatment for one may kill another.

I didn't say they did exactly the same things. I said that they were the categories that can be directly compared, rather than comparing different subsets within each. Just as a truck, a car and a motorcycle can be compared as complete vehicles, not truck axle vs car engine vs bike wheel.

The common denominator of LC and the original ME archetype (CVB/EV-Bs) is Pisoniviricetes-induced disease, the ICTV 2025 virus class that both belong to. This disease category includes COVID, EVID, NOVID (NoV: norovirus), etc. EV-ME is part of EVID, as are the EV-induced cases of other diseases.

By 1994, they were already ignoring the acute "myalgic" + "lymphoreticular" + "encephalomyelitis" phase of the disease, which subsides from the outbreak form over weeks or months to a more heterogeneous mix of sequelae that may include POTS, MCAS, fibromyalgia, Sjögrens and other overlapping terms.

ME & ME/CFS have become idiopathic terms, having lost their associations with their specific pathogens over the last 4 decades. Without knowing what's there, it's impossible to know whether or how an intervention will affect a specific case. Suppress one, aggravate another, remove a support, etc.

Crystal @gemzme.bsky.social · 11/09/2026
PEM was a poorly defined term, often described as feeling a bit off (possibly apt for the mild cases). The term has evolved beyond the original scope to describe what's seen in or experienced by cohorts and cases.

Crystal @gemzme.bsky.social · 11/09/2026
Few, if any, organisations have institutional memory of ME. Mostly, they were influenced by CFS, subsequent and hybrid iterations. Modern usage of the ME splits between the "sounds more scientific" adoption to describe the diagnostically drifted and hybrid ME/CFS versions and diluted original ME.

Crystal @gemzme.bsky.social · 11/09/2026
If you remove the EV-Bs, myalgic, lymphoreticular and encephalomyelitis/encephalomyelopathies from ME, you get idiopathic MCAS, dysautonomias, SFNs,…reactivated HHVs, and many other signs, symptoms, conditions typically induced by EV-B infection that can be reframed as fatigue, & CFS after 6 months.
010
Crystal @gemzme.bsky.social · 11/09/2026
ME was inconvenient. Non-polio enterovirus-induced diseases were inconvenient when the world was eradicating poliovirus-induced paralysis & being forced to recognise the rise of HIV/AIDS. Far better to wait until the acute stages had subsided and the remainder could be framed as fatigue or hysteria.

Crystal @gemzme.bsky.social · 11/09/2026
CFS was deliberately created to be different from ME: different causes, different timings, different criteria. Later conflation led to ME/CFS and CFS/ME. It's possible to have CFS (sequelae from the ME infection or comorbid from other causes) after ME, and CFS without ever having had ME.

His 2018 tweet is consistent with the 1980s diagnostic shift from "outbreak" ME after (mostly) EV-Bs to idiopathic post-acute CFS.

It's not as different as it's portrayed and can be grouped as Pisoniviricetes-induced disease, Pisoniviricetes being the viral class that includes the orders Picornavirales and Nidovirales responsible for ME, COVID and many more diseases. What's missing is any serious attempt to deal with the class.

They are, but these affect people differently according to viral strain and the host's age & sex. Teen girls are in the first peak of ME. In utero affects development and survival even more than the neonatal myocarditis/sepsis, while men are at higher risk of dropping dead during strenuous exercise.

The original ME was deemed most likely to be [Enterovirus betacoxsackie (EV-B), the current name for Coxsackie Bs, Echoviruses, and some others]. The modern variations of ME/CFS include EVs, HHVs, and other infections as well as non-infectious onsets. In both cases, it can involve metabolic effects.

Prior to the SARS-CoV-2 pandemic, the acronym COVID stood for coronavirus-induced disease (a category that included any and all coronavirus-induced diseases). Using this definition, Long Covid would include all the long-term coronavirus-induced diseases and their component signs, symptoms, etc.
