![Hazel has demonstrated adequate knowledge of the benefits and potential risks of gender affirming orchiectomy, as well as the potential negative and positive outcomes, including the length of recovery, specific post-op instructions to be followed during this time, as well as the permanent and irreversible nature of the procedure, risks associated with fertility and sexual health, including loss of fertility. It should be noted she has been living in a gender role congruent with her gender identity since 2023 and has been utilizing gender affirming hormones since 2022. It should also be noted that she has identified [redacted] as the specific support persons who will assist her pre-op and post-op and she will be recovering in [redacted] for two days before she is medically cleared to return home to [redacted].
Additionally, Hazel is psychologically stable for gender affirming orchiectomy. There is no evidence of any symptoms of psychosis or disturbances in personality. Neither tobacco use or substance use is of concern and she currently denies any suicidal ideation, plan, or intent. As such, no significant medical or mental health concerns are present, and her symptoms of anxiety/depression are reasonably well controlled. The decision to have surgery is current, well thought out, not impulsive, and not due to any other treatable mental disorder other than gender dysphoria. I believe Hazel will continue to benefit greatly both medically and psychologically from gender affirming orchiectomy. I have also made myself available to the patient for any psychological support she may need.
Overall, the patient is capable of giving informed consent, and my assessment deems the patient appropriate to undergo gender affirming orchiectomy. Please also note, the client has experienced no coercion from external parties and provided informed consent to move forward with surgery associated with the procedure. Given her insight and judgment are sound, it seems likely any prescribed medication, pre and post-op recommendations, and long-term recommendations will be followed in a responsible manner. She can comply with long-term follow-up requirements and post-operative expectations have been addressed. I hereby recommend and refer, Hazel.
