#worldsepsisday April 2025 I was fit, training for a 2nd ultramarathon. I felt flu-y, 48h later in hospital. 24h later I was peri-arrest. I was delirious and struggling to breathe. 2 weeks later I left hospital and began the long road to recovery that I’m still on. #sepsis can happen to anyone.

As I anaesthetise (almost) exclusively #cancer patients, a large proportion of whom have had steroids and some are adrenally insufficient due to immunotherapy this is my most well used guideline.

This year my boys decided that Santa wanted a shot of lemon gin (straight, no tonic 😬), a gluten free mince pie - homemade because Santa likes homemade better and would leave more presents and the obligatory carrot for the reindeer. My son became good friends with a boy who spoke little English at nursery. One day his parents returned permanently to Ghana - we didn’t know they were leaving. For months afterwards my son asked me to take him to Ghana so we could find the ‘blue school’ that his friend was going too.

In the UK surgeons take longer purely to annoy the anaesthetist; working in the NHS there’s no financial gain to prolonging surgery so surgeons might as well have some fun. Well, unexpected complications, bleeding, adhesions etc. can occur… but that’s just to make us stop playing wordle 🤣

What is a ‘safe dose’ of LA? I read an article when training on how we should avoid using traditional ‘safe mg/kg doses’ and think more about the site, the patient, co-morbidities etc. I can’t remember where I read this but it’s something that’s stayed with me. Even so, this case is a rare event!

Interesting work. Just reviewed the evidence on TXA and onco-Anaesthesia for an article I wrote. No evidence of increased thromboembolic events and in certain surgeries decreases blood loss. Yes I think we should be looking at how we get regular TXA use implemented. Well done @swarnak.bsky.social 👏

Well said. ‘Banter’ when there is inequality in status or power isn’t really banter. It is not pleasant to be on the receiving end of this and the power differential makes reporting it very difficult.
