Healthcare is becoming corporate. But medical school still teaches doctors nothing about business or how organizations actually work. Then we wonder why doctors and corporations clash. Doctors are trained for medicine. Corporations for business. The mismatch is structural. 🎯

Fluency is not accuracy This study is a great reminder: polished AI radiology reports can still be diagnostically poor On the positive side: the only startup-developed VLM outperformed models from Alibaba, Microsoft, Google DM & academia VLMs will become standard & startups will keep leading 🚀

2026 is a turning point. Claude Code showed AI could do the work, not just assist. Healthcare is next. Radiologists won’t read → dictate → edit. We’ll see → review → validate. The workflow changes. The profession follows.

We spent years worried AI would replace radiologists first. Meanwhile, everyone’s using ChatGPT as their primary care doc. Primary care runs on high-volume, low-complexity visits. That’s exactly what AI is best at. GPs didn’t see it coming. Neither did we.

In healthcare, you only understand how game-changing AI is once you use it. What strikes me most is how reliably opinions flip. The skeptic radiologist goes from “I don’t need this” to “I can’t imagine working without it.” Not occasionally. Almost every time.

Every time I open a radiology case, I wonder: why do we get the date of birth… but not the patient’s age at the exam? Why make reporting more complex than it needs to be? Or is it a benevolent way to keep radiologists sharp with daily mental math?

In overstretched radiology departments, improving diagnostic accuracy with AI is not enough. We also need measurable efficiency gains. This study from my colleagues at 3R shows the way: +13.4% reporting speed for knee MRI with full AI integration and auto-filled reports. 🚀

A lot of radiologists I’ve met genuinely like routine exams: simple, predictable, low-risk. I never did. Routine bores me. Whenever I’m doing repetitive work, my immediate thought is: a machine should be doing this.

Radiologists are backstabbed every day. While you’re reading 1 case, the universe is quietly loading 10 more in your worklist. You never catch up because unlike other doctors, your worklist is disconnected from patient flow; imaging is done in parallel, and you sit at the end of the funnel.
