In this episode of Cancer Questions with Dana-Farber, Mark Pomerantz, MD, discusses the complexities of prostate cancer screening. The American Cancer Society recommends that men over 50 with a 10-year or more life expectancy consider PSA screening and consult their primary care doctor. Screening is especially important for men with a family history of the disease and men of West African ancestry, who should begin screening in their 40s. The goal of screening is to identify aggressive cancers while avoiding unnecessary treatments for low-risk prostate cancers, which are often managed with active monitoring. My name is Mark Pomerantz. I'm a medical oncologist at Dana-Farber. I'm with the general urinary oncology division. Today's question is: should I get screened for prostate cancer? The issue of prostate cancer screening has been controversial for decades. Prostate cancer is the second leading cause of cancer death among men in the United States. At the same time, most prostate cancers are harmless. The American Cancer Society recommends that all men over the age of 50, with a ten-year life expectancy or more, should strongly consider PSA screening and should discuss the prospect of screening with their primary care doctor. There are two groups in particular that should be getting screened: men with a family history of the disease and men of West African ancestry. Screening should begin earlier in their 40s. Prostate cancer is exceedingly common. If we go looking for prostate cancer, we're going to find a lot of prostate cancer. And this has consequences. It could lead to unnecessary treatments that have lifelong side effects. So, our goal has been to identify the dangerous cases while avoiding the prostate cancers that will never cause harm. Most men with low grade, low volume prostate cancer are referred for active monitoring, with blood tests, imaging and biopsies to make sure that they have a prostate cancer that does not deserve aggressive treatment.
