People tell us that their wishes for the end of their life are often frustrated by a medical culture that defaults to interventions and hospitalisations, without prioritising if the person wants this. The impact is devastating. They endure unwanted treatments and feel powerless over their care.

The new Modern Service Framework for Palliative and End of Life Care; the neighbourhood health agenda; and work on the NHS Single Patient Record are important opportunities to improve the ability of the system to listen and honour peoples end of life decisions. It’s vital that we get this right.

Too often, people and professionals are trying to have the same conversation from separate rooms. Whilst NHS increasingly says end-of-life care should be personalised, less hospital-based and built around people’s wishes, in Anushka’s experience, the system is not designed to respond in this way.

We heard from the professionals we interviewed about a form of “death denial” in professional culture, which is not primarily personal but deeply embedded in the structures that underpin healthcare practice. It is reinforced by training, hierarchy, system pressures and fragmented record systems.

Professionals described environments where time for meaningful conversations are limited, accountability mechanisms reward action over restraint and ‘doing more’ feels safer than avoiding unnecessary treatment – even when clinically justified and aligned with what matters to the person.

It shows that people’s wishes for the end of their life are frustrated by a medical culture which defaults to tests, interventions and hospitalisations, without prioritising whether individuals themselves would want this. But professionals told us the problem is rarely a lack of compassion.
