tl;dr it's probably not as significant as people think and it's very normal to have spontaneous breast growth in 5 years into HRT. i went from a C cup to an E cup on years 5-9 of HRT. nothing changed in what i was doing, and i have never smoked or vaped. i wouldn't smoke or vape mostly b/c it's Bad.

people who smoke & vape have similar depletion of vitamin C, in example, and is also an essential co-factor for collagen biosynthesis. collagen is a critical structural component of our breast tissue, with the stroma of the breast. smoking obliterates folate, b6, b12, calcium, and vitamin D levels.

so, can't really say how much vaping influences breast growth. what is true, however, is that smoking and vaping cause nutritional deficiencies / depletion in multiple micronutrients. people who vape and smoke have also poorer overall diets, and weigh ~8-12 lbs less than non-smokers similar to them.

and, the capacity of nicotine to interfere w/ the estrogen receptor is not really elucidated in literature, but we know it happens because of bone density changes, hot flashes, urogenital symptoms, and lipid metabolism worsening, specific to how estrogen influences those. vaping does it less worse.

nicotine can also reduce the capacity to do SHBG-binding in the body, through the same mechanism of the liver. it is dose-dependant in all of the circumstances, so people who are smoking a cigarette or three won't really notice much. if you're doing a pack a day, though, very much so.

it's a 50% reduction in circulating estradiol in people who are smoking 15+ cigs a day- the 2-hydroxylation activity is ~53% higher in smokers smoking such versus ~35% in non-smokers. what nicotine [and cotinine, the metabolite of nicotine] does, is interfere with estrogen receptor signalling.

it is not nicotine per-say but the induction of CYP1A1 and CYP1A2 from polycyclic aromatic hydrocarbons in combusting tobacco. the increased hydroxylation of estrogen in the liver creates 2-hydroxyestrogen, which is cleared much quicker in circulation and of which has minimal estrogenic activity.

here in north america whether you're here in canada or in the US, basically all of our insulin comes out of north carolina. france, germany, mexico, and india are the other large places of manufacturing and insulin exports. the US government would also just nationalize any problematic corporation.

in example: a common way of inducing lactation in women is through a gut motility agent called domperidone. domperidone does not cross the blood-brain barrier, but antagonizes dopamine receptors in our gut, to increase gastric emptying. when the body detects this- it nevertheless raises prolactin.

for the same reason people in general can and will lactate from antipsychotics, calcium channel blockers, protease inhibitors, or even opioids / cocaine. lactation happens -after- pregnancy*, not -during- it. after progesterone levels drop, a dopamine-prolactin circuit triggers; you lactate.

good example is GHB. GHB is common to see recreationally in MSM. all you hear w/ GHB though is that it's used to "spike" drinks / is used for date rape. this is an urban legend like fentanyl in weed / hasn't been validated in literature. most assault is acquaintance-related, and via alcohol.

part of harm reduction for me is communicating accurate information, basically- it doesn't help that poppers historically have been maligned in medicine due to stuff involving gay men's sexuality and HIV/AIDS, that ends up seeming more on the side of homophobic propaganda than scientific inquiry.

yes, the increased risk observed is a confounder specific to this population and why specifically they're being prescribed these medications- for angina, and the underlying issues which causes angina. they are not suddenly dying as is claimed in the original tweet. it's important context.
