Let's start with the fact that Trump's personality precludes him from having anything approaching an actual sense of humor. He says things that he actually means...intentionally or otherwise. He clearly meant that, if Iran had the capability, he would be OK with LA or San Diego being ICBM targets.

Thinking that both "fascist" and "communist" have been hurled as hyperbolic political epithets between Americans, mostly since the end of WW2. Since the 1930's, the two worst things that an American could be accused of being are a "fascist" (especially "Nazi") or "communist" (Leninist, Stalinist).
