Here's the orthodoxy according to Ad de Jongh: NICE says psychological treatment for personality disorders should last at least a year. Yet almost half of patients don't respond well and about a third drop out. What if shorter, targeted therapy could work? #BIGSPD26

There's little evidence that DBT, MBT or Schema Therapy beat treatment as usual, and no evidence that longer treatments beat shorter ones for personality disorders, says Ad de Jongh #BIGSPD26

Why focus on trauma? Childhood adversity isn't just a risk factor for personality disorders, says de Jongh, it may be a central driver. In one study each ACE raised borderline PD risk fivefold. Emotional abuse and neglect were the strongest predictors. #BIGSPD26

An intensive 8-day trauma-focused programme (prolonged exposure + EMDR) for people with PTSD and borderline PD. One year on, 73% no longer met criteria for borderline PD. Dropout just 1%. We couldn't believe our research results, says Ad de Jongh #BIGSPD26

We need to focus on Adverse Childhood Experiences, says de Jongh. Using Shapiro's Adaptive Information Processing. A negative self-image like 'I am worthless' isn't the cause of dysfunction but the result of unprocessed memories. Treat the memories, shift the symptoms. #BIGSPD26

The TEMPO study looked at EMDR therapy for people with personality disorders, with and without PTSD (N=159). Remission of PD diagnosis rose from 44% to 65%, with a 5% dropout. #BIGSPD26 pmc.ncbi.nlm.nih.gov/articles/PMC...

Introduction: ‘You probably weren't born with these complaints, right?’

• NEGATIVE BELIEFS: Negative core beliefs and self-image issues (e.g., "I'm worthless")
‘What made you believe you're worthless?’
‘What experience currently (still) emotionally proves to you that you're worthless?’

• NEGATIVE FEELINGS: Feelings of emptiness, loneliness, or suicidal thoughts)
‘What caused this feeling?’ ‘What events have further exacerbated this feeling?’

• ANTICIPATORY FEAR: Fear of the future (e.g., fear of rejection or fear of abandonment)
‘What's the worst that can happen?’ ‘What does that disaster fantasy look like in your head?’

• FUNCTIONING IN INTERACTION WITH OTHERS: (e.g., not trusting others)
‘What happened that made you not trust others ‘What situations from your early childhood prove to
you, emotionally, that you can't trust people?’

• CONTROLING IMPULSES AND EMOTIONS:
‘What happened that made it difficult for you to control your emotions?’
‘What image should you conjure up that makes you feel that fear/anger/sadness/guilt to the fullest?’

This is Ad de Jongh's big question for #BIGSPD26 Can short-term trauma-focused treatment help people with a personality disorder change? His answer: yes. Brief, intensive care delivering large and sustained gains, and a challenge to how we commission 'personality disorder' services.

We are now listening to Ad de Jongh, Professor at the University of Amsterdam. The man has written over 500 papers!!! His #BIGSPD26 discusses how CPTSD is a strong crossover with #PersonalityDisorder. He worked with a service to research and published.

Annette’s Acute Therapy Service manages to create change within a short time frame, decreasing bed days and proving a huge injection of understanding and validation to the service-users they serve. Their patient feedback shows the relational aspect is the most important. #BIGSPD26
