Reforming global health for an age of mistrust, fragmentation and overlapping crises - Nature Health
To move from debate to delivery, global health architecture reform must rebalance power, reinforce systems and restore trust, with a sharper focus on managing geopolitics and emerging threats.

The true test of reform is not whether it satisfies those who established the current system, but whether it serves those without whom the current system was designed. Half the world still lacks access to essential health services. Tomorrow's institutions must close this gap. 🔗: www.nature.com

The subordination of health to short-term defence, trade, and foreign policy priorities necessitates a broader forum for rethinking. Health reform disconnected from the geopolitical forces that shape it will be rendered irrelevant by them. 🔗: www.nature.com/articles/s44...

Global health architecture reform should be built to navigate an era of fragmentation rather than to overlook it. In practice, this means designing reforms that can function even when universal consensus fails. 🔗: www.nature.com/articles/s44...

On rebuilding trust: trust receives little attention in reform discussions. Without it, rebalanced power becomes contested legitimacy and reinforced systems become empty infrastructure. From the confronting the DRC Ebola outbreak to bilateral transactionalism, rebuilding trust is crucial. 🔗:

On reinforcing systems: institutions are built to solve yesterday's emergencies. Donors prefer quick wins. Recipients accept misaligned aid to avoid losing funding. Governance rewards self-preservation over collaboration. A political economy that rewards fragmentation must be confronted. 🔗:

On rebalancing power: sovereignty in global health is not the rejection of cooperation — it is the precondition for participating in it as an equal. Yet health decision-making sits far from communities. Regional orgs and nonstate actors remain excluded despite being essential to delivery. 🔗:

Elders warn against bilateral pandemic deals that could impact multilateral cooperationElders’ Statement | 21.11.2025

“Parallel systems created through bilateral agreements should not undermine the foundations of global preparedness and response being built at the multilateral level. Once multilateralism is hollowed out, it is hard to rebuild...Decisions taken now will determine whether the world can build a future of shared safety and collective action, and whether all people are protected from pandemic threats.”
