To celebrate #BTS coming home, #BAMUHCP & #BAMUNursing are highlighting discharge instructions. When you/a loved one is admitted to a hospital, the ultimate goal is to get HOME safely. We're making sure you have all of the tools to successfully (and safely) return HOME🏠.

2. Medication Guidance. Know what medications you need to take, why, when, how, possible side effects, & how to dispose of unused medicines.

4. Post Discharge Care. Know what kinds of follow-up care you will need, such as physical therapy, & what medical equipment you will need, and how to get it.

6. Stay Informed. Know your medical condition at the time of discharge, when and how you will receive test results, & what to expect at your new facility if you’re not going home.
