Thrilled to announce our latest ACP study, conducted across 50 clinics in the UC health systems. We found that adding health navigator outreach to PREPAREforYourCare.org and mailed materials substantially increased AD/POLST documentation for seriously ill patients. pubmed.ncbi.nlm.nih.gov/41285010/
