@walkeyallan and I wrote a letter. Our point: Jubran et al report that 53.7% of 315 LTAC patients were disconnected from the ventilator. Go to reference 4 to find the whole cohort diagram - these 315 pts were previously selected from 2267 pts for an RCT. bit.ly/2TzVQU5

Mike Klompas makes a thoughtful argument here: put the brakes on giving empiric abx to patients who may (but also may not) have sepsis if they don't also have shock (as it is less clear that rapid abx use matters as much for those without septic shock)...
