I'm biased obviously, and I know time is an issue but I'd recommend CBT for either issue if possible in the future. I do think your request sounds entirely reasonable though, and other than cost, I'm not sure of the rationale for one month. Sertraline isn't an expensive drug over here.

I've not heard of that policy, unless part of a planned review. If access to Sertraline in USA isn't possible however, you risk some possibly unpleasant side effects from 'cold turkey' discontinuing. That would be my argument for having a larger supply.

I can confirm that this is a remarkably enjoyable night. We didn't know what to expect but followed a Mark Thomas recommendation. I'm not sure my brain managed to keep up with everything on the train monologue but regardless, we found it funny, fulfilling and insightful.

I think I'll just reiterate what I've said. I think it's reasonable to have a high expectation of therapists to walk the walk.

You can't encourage anyone to do something you wouldn't do yourself. It's a fundamental rule that I'm sure my fellow trainers would agree on.

Humility is often mentioned as 𝘢𝘯 important quality in a therapist, but given its impact on the other virtues, it might be 𝘵𝘩𝘦 most important quality in a therapist.

Well CBT Blue Sky, it's finally happened, I'm teaching a new cohort of high intensity trainees that contains individuals too young to know who Columbo is. It's a sad day indeed. Thoughts and prayers. #onemorething

I've never dealt with the UKCP but the notion that as a therapist you are free to impart your beliefs and values onto a potentially vulnerable patient really doesn't sit well with me.

Best advice I never took was when I was 30 and on a massive career fast-track: You cannot cuddle with your career on a lonely night when your life is on the decline. Invest as much time on love and family. Sounded twee then. At 62 and near retirement, I now know it's 100% true.

I think the last time i saw stats, it was about 3% survival, which should measure expectations, but that's still 3 in 100 people that'll be glad you tried.

Alex Preston @alxprestoncbt.bsky.social · 02/09/2025
I would agree, to a degree, with the caveat of course that CBT doesn't have any ambitions, it's not an entity with drive or a wish for dominance. It's just a therapy that is being shoe-horned into areas that any experienced therapist would be resistant to.

A reminder that the acorn of this theory is with a hideous individual that made it all up, and was struck off the medical record. It was all utter BS. Had we found that immunisation caused autism, we'd actually understand a lot more about autism than we do now. And we'd welcome that knowledge.

I believe i shall be 'borrowing' this for next year's therapeutic phenomenology section. Thinking about how this might ring true for therapy also, maybe the only difference being the latter is how it pertains to the person and this quote, how it pertains to people? (With some overlap)

I don't know the situation but if that brief detail is accurate then it's just a poor assessment/judgement call. Stepped care is still congruent care, it's not supposed to be an arbitrary hurdle.

Alex Preston @alxprestoncbt.bsky.social · 13/08/2025
Depending on severity of issue, people may miss the low intensity step all together. This info isn't my opinion as an educator in this area or therapist, it's the model the services are designed on. If it's not happening then please complain, as it's just wrong and those of us in service hate it to
120
Alex Preston @alxprestoncbt.bsky.social · 13/08/2025
Agreed once more. Services are supposed to offer the least intrusive support, which initially often means low intensity CBT (or management strategies - 6x30min sessions) but then if that doesn't work people should be stepped up to high intensity CBT which are hr long sessions up to 16 sessions.
010
Alex Preston @alxprestoncbt.bsky.social · 12/08/2025
I think I'd agree with your disagreement. We know indications from research as to what timeframe might be necessary (for specific issues), but it's a poor therapist (or more usually service manager) that thinks that's an absolute, or that clinical trials are the same as real life.

Was reminded of Bentham’s argument that fear of crime can be a worse harm than crime itself. Everyone can be affected by fear of crime but relatively few people experience serious crime. And fear of crime can easily be manipulated for nefarious purposes.

There is nothing wrong with being a Freudian, a relationalist, an interpersonalist, a self psychologist, or whatever, as long as one does not succumb to the delusion that their school has a monopoly on truth and there is nothing to be learned from other approaches. - Arnold Richards
