Has anyone seen full-dose SC unfractionated heparin (333 units/kg load, then 250 units/kg Q12H) used for VTE treatment in non-hospitalized pts? I haven't seen it before. Still get asked about Paxlovid for covid at times. Most pts don’t need it especially post-vaccination. EPIC-SR trial showed no significant benefit in reducing hospitalizations or deaths. With lower covid morbidity & immunity from vaccines & different strains, Paxlovid offers little to no benefit☹️ 1/

Not all aspirations need Abx ☺️ aspiration pneumonitis resolves in 24-48h; if symptoms persist >48h, treat aspiration pneumonia with CTX CTX covers key oral anaerobes (eg Peptostreptococcus) so extended anaerobe coverage is usually unnecessary avoid piptaz first-line & reserve for resistant cases
