Great educational case! The correlation between FFR and resting gradients is ok, but far from perfect (R values ~0,8). The answer to this case is nicely demonstrated in the paper below: Resting gradients tend to underestimate focal stenosis due to low friction. doi.org/10.1161/CIRC...
