The first Ebola case has been reported today in France. Our rapid risk assessment last month showed that France consistently ranked among the European countries at highest risk of importation if an Ebola case were to arrive in Europe from the ongoing Bundibugyo outbreak.

Supporting outbreak response where transmission occurs remains the most effective strategy both to reduce the local burden of the epidemic and to limit international spread.

But broad travel restrictions may also have unintended consequences by isolating affected countries and hindering the mobilization of healthcare workers, medical supplies, and intl support - concerns raised during the 2014 Ebola epidemic and readily emphasized today by AfricaCDC.

Such modest delays may provide valuable time to strengthen preparedness, surveillance, and response capacities.

Modern transportation networks further amplify spatial spread because they are highly heterogeneous: a small number of major hubs concentrate a very large share of global connectivity. www.nature.com/articles/nph... journals.aps.org/prl/abstract... www.sciencedirect.com/science/arti... @alexvespi

Additional travel-related measures (screening, testing, quarantine, traveller monitoring) may contribute to reducing importation risk under specific conditions, but their effectiveness depends strongly on operational implementation and pathogen characteristics.

Mathematically, the delay increases only logarithmically with the magnitude of travel reduction — so very large reductions produce only modest delays in international spread. www.nature.com/articles/nm0... www.sciencedirect.com/science/arti...
110
Vittoria Colizza @vcolizza.bsky.social · 25/05/2026
Why? Because once sustained transmission is established in the epidemic source area, cases grow exponentially. Even if travel volumes are drastically reduced, the rapidly increasing number of infections at the source continues generating some infected travelers.
100
Vittoria Colizza @vcolizza.bsky.social · 25/05/2026
However, theoretical, computational, and empirical studies consistently show that travel restrictions alone generally produce only moderate delays — typically on the order of days to weeks.
100
Vittoria Colizza @vcolizza.bsky.social · 25/05/2026
This may seem counterintuitive. Infectious diseases spread through human mobility, so one may expect that strongly reducing travel would prevent international spread.
100
Vittoria Colizza @vcolizza.bsky.social · 25/05/2026
Our analyses showed that once sustained transmission is established in the epidemic source area, even large reductions in travel still allow some infected individuals to move internationally. www.eurosurveillance.org/content/10.2... @chpoletto.bsky.social @alexvespi.bsky.social

Eurosurveillance | Assessing the impact of travel restrictions on international spread of the 2014 West African Ebola epidemic
The quick spread of an Ebola outbreak in West Africa has led a number of countries and airline companies to issue travel bans to the affected areas. Considering data up to 31 Aug 2014, we assess the impact of the resulting traffic reductions with detailed numerical simulations of the international spread of the epidemic. Traffic reductions are shown to delay by only a few weeks the risk that the outbreak extends to new countries.

Model results aligned with post-outbreak survey data (ERAS 2023) reporting that ~46% of MSM reduced sexual partners during that outbreak.

Instead, the decline was best explained by widespread behavioural change, adopted by ~half of MSM, *independently of individual risk level*. This prevented an estimated ~70% of potential cases during summer 2022.
