Sometimes the answer is "both." Collaborative Care bundles clinical interventions with implementation strategies that are hard to pull apart. Some mHealth apps may be inseparable: on-demand access raises reach, and the same on-demand quality may act clinically through self-agency.

Alternatives the paper considers and sets aside: who's targeted, who delivers it, whether it needs its own implementation support, whether it moves clinical outcomes. Implementation strategies can target patients, and can move clinical outcomes indirectly or as treatment effect modifiers.

The proposal: classify by mechanism of action. Clinical if it works through a patient's biological or psychological target. Implementation if it targets an implementation barrier. Ambiguous? Look at the proximal outcome: clinical (NIH/FDA) or implementation (RE-AIM, CFIR, Proctor).

Why it matters: the classification sets your primary outcome, your comparator, and your decisions about site selection, eligibility, masking, and fidelity. It also decides whether your hybrid trial is type 1, 2, or 3, which reporting framework you use, and whether reviewers can pool your findings.

We wrote this for research teams, and we hope it is also useful for the funders who shape what gets proposed and evaluate what to fund. In our pilot mechanism, pre-consultations with HCD and IS experts and continued support throughout the review and project process helped integrate perspectives.

The rest are questions for your team: what roles do HCD and IS play? What are your design problems? Which methods fit? What expertise do you need? How much decision-making will you share with users? How will you measure success? How will you catch and address unintended harms mid-project?

The alternative: ground the uncertain parts in what you know, name the decision points, say what you'd do for some possible outcomes. Our TF-CBT proposal listed plausible solutions without committing to them. Co-design participants proposed several, plus ones we hadn't imagined.

The failure we see most: a proposal claims HCD, but the team has already chosen a solution, without the user research or evidence to back it up. If user research is part of the proposed work, the proposal needs to show how you will listen to that research in subsequent steps.

The hardest problem is uncertainty. In HCD you don't know the design at the outset; you know the process for arriving at it. Collaborators and reviewers who expect a defined endpoint find that unnerving.

HCD helps make an intervention usable and worth adopting. IS works out what it takes for it to reach people and last. More teams want both, and existing IS proposal guidance doesn't cover what happens when you put them together.
