It does not I made sure of that... as in I just checked. Some people might know my main after this but I dont care its important that people know that no, im not collecting anyone's e-mail. My best guess is Google being Google or the image uploaded that uses GDrive.

A picture of the Forms configurations, in it shows two active points one saying "Collect e-mail adresses" with the option being put in as "Dont Colect". Its all in Portuguese and you can look it up and translate it yourself.
