Victor happens to be a lot more aware of being a fully separate being from his body parts than initially thought. The last line does a lot of heavy lifting to imply that, while the core of his being *is* still inside the torso, he's very aware that it wasn't meant to be there.

I think the muzzle drifter is talking about with regards to shiv is his reliquary. if shiv is a vampire the society probably wanted some kind of reassurance that he wouldnt start biting people (MUZZLE) so his reliquary drip feeds him blood. his rage is actually his true strength
