Calling them slurs doesn't hurt them, they brush it off, because the institutional foundation of what the slur insults is on their side. Calling them slurs with intent to insult them reinforces the idea that the meaning of the slur is insult worthy. We want them to stop using slurs.

They can't easily tell these phenomena apart because everyone is serious when seeing a doctor. If they don't like being able to describe your symptoms with more complexity than "head hurt" that's often an ego thing. Seen more in GPs and specialists than like ER staff in my experience.
