I’ve also read a lot of scripts recently where someone has CPR and then, when their heart restarts, they’re remarkably chipper about, well, having died and had their chest brutalised! People take a while to recover from CPR! (9)

Another reason CPR is exhausting is because of the depth you need to go. Recommendations in adults are 2 – 2.5 inches (5-6cm). It’s essential to allow the chest to recoil again after a compression, otherwise the heart won’t be able to refill. (8)

CPR should be performed with interlocked fingers, straight arms, using the heel of the hand to compress over the lower part of the sternum (breastbone). The rate of CPR is recommended at 100-120 compressions per minute – ‘Stayin’ Alive’ is the classic tune that’s used! (6)

I’ll start with some basics. CPR should be started when there’s no discernible pulse, or certain rhythms (asystole, PEA, VF or pulseless VT). Before starting CPR, open the airway with a ‘head tilt, chin lift’. Rescue breaths at the start can be given in kids or after drowning. (5)

Ultimately, my gripe is this – you never shock a flatline. Ever. Because it does nothing. Your patient is dead, and you need to restart their heart, not randomly send electricity through them. Call for help, start CPR, then treat as per ALS protocol. (8/9)

A couple of other points – if you’re defibbing a patient, there should be no open oxygen circuit within 2 metres of the patient, nobody should be touching the bed (otherwise you can shock them too!) and the body doesn’t jump off the table. You’re only using normally 120-200 joules! (7/9)

Some rhythms which are ‘shockable’ include ventricular fibrillation (VF) and pulseless ventricular tachycardia (VT). In these cases a rhythm is present, but it’s not compatible with proper cardiac output, so the defib stops the bad rhythm and lets the pacemaker take over again (6/9)

However dramatic it is for someone to come across, scream “CLEAR!” and zap them back to life, there is no rhythm to correct in the patient. This lack of activity responds to CPR and adrenaline, however prognosis is poor (success rate between 5-15%) – I’ll discuss CPR another time (5/9)

Below is asystole (or ‘flatline’). Present in approx. 30% of hospital cardiac arrests, it shows a lack of electrical cardiac activity. This is the time in a film when all hell breaks loose, crash buzzers go, there’s loads of shouting and drama, accompanied by a very loud ‘BEEEEEEEP’ (4/9)

Before I go into what rhythms respond to a defib, let’s quickly look at a normal heart tracing. Below is a normal ECG. The P wave is the heart’s pacemaker kickstarting the electrical activity of the heart, the QRS complex is the muscular ventricles contracting, and the T wave is them relaxing (3/9)

Defibrillation is the delivery of electrical current to the heart in an attempt to stop unusual rhythms. If the heart is in a bad rhythm, it pumps poorly, meaning blood doesn’t get to where it needs to go, and all of our organs most definitely need blood (2/9)

Besides writing and doctoring, my other passion is good beer. Here's tonight's offering - a pineapple, yuzu and lychee sour with cardamom from Pomona Island. Fiercely sharp tartness, beautiful pineapple and lychee sweetness and the cardamom is a welcome warmth at the end. Belies its 6.5% ABV 🍻
