And what they can't collect is mostly owed by people without insurance. Insurance companies typically write off a large percentage of the charges because they know the charges are excessive. Private payers don't have that option.

If you honestly can't pay most hospitals will write off the debt because trying to collect it costs money too. Those costs aren't necessarily absorbed. Often they're just passed on to people who can pay them by increasing the prices they charge. Twelve dollars for 325mg of Tylenol instead of six.
